氨溴索联合NPPV对AECOPD气道炎症细胞及血清炎症介质的影响

摘　　要：目的探讨氨溴索联合NPPV对AECOPD伴Ⅱ型呼吸衰竭患者气道炎症细胞及血清炎症介质的影响。方法病例来自我院呼吸科住院30例,分为治疗组和对照组,每组各15例。两组给予抗感染、支气管扩张剂、祛痰剂基础治疗,同时使用NPPV通气支持治疗,每天10小时;治疗组加用盐酸氨溴索60 mg雾化吸入2次/日,疗程7天,观察诱导痰中性粒细胞(NEU)及血清肿瘤坏死因子a(TNF-a)变化。结果治疗组、对照组治疗后,总有效率82.6%、65.1%,治疗组有效率高于对照组。在改善临床综合评分治疗组优于对照组。治疗组诱导痰中性粒细胞数量下降P<0.01、血清TNF-a治疗组前后相比P<0.05。结论盐酸氨溴索雾化吸入联合NPPV治疗AECOPD伴Ⅱ型呼吸衰竭有明显的疗效。Objective To investigate the impact of ambroxol combined with NPPV on inflammatory cells in airway and serum in- flammatory mediators in patients with AECOPD complicated with type lI respiratory failure. Methods 30 cases from our Department of Respiratory were divided into the trial group and the control group, 15 eases for each. group. Patients in the trial group were treated with anti-infection, bronchodilatnr and expectorant together with supportive NPPV ventilation treatment, whereas patients in the control group were treated with 60rag of ambroxol by aerosol inhalation twice a day for 7 days. Then, we observed the induction of neutrophils in phlegm and the change of serum TNF-α. Results After treatment, the total effective rate for the trial and control groups are 82.6% and 65.1% respectively. Moreover, the trial group has improved clinical integrated scores if compared with the control group. The decrease of neutro- phils in phlegm （P 〈0. 01 ） and TNF-α （P 〈0. 05 ） in trial group after treatment have statistical significance as compared to the control group. Conclusions Aerosol inhalation of ambroxol combined with NPPV treatment have the obvious curative effect for patients with with AECOPD complicated with type Ⅱ respiratory failure.
