皮肤软组织扩张术治疗烧伤疤痕的临床应用

摘　　要：目的:探讨皮肤软组织扩张术在治疗不同部位疤痕的方法、并发症的防治及临床效果。方法:根据不同部位及疤痕情况,选择正常皮肤切口,选择合适的扩张器植入,少量多次注水持续扩张,通过推进滑行、皮瓣转移等方法,合理设计利用扩张皮瓣修复创面。结果:本组24例患者,术后扩张器外露1例致扩张失败,术后并发出血致腔内血肿2例,经血肿清除后继续扩张达到了扩张效果,其余病例均达到了理想的扩张效果,修复满意。结论:根据不同部位及疤痕情况,选择正常皮肤切口,选择合适的扩张器植入,注意并发症的防范,皮肤扩张后合理设计利用是手术成功的关健。Objective To investigate the methods of treating different scars, the ways of preventing complications and clinical effects. Method According to different body area scar ,we chose normal skin incision and transplanting suitable skin and soft tissue expand- er, and continued expand with injecting normal saline, then repaired the defects with rotation or slid flaps. Results Of total 24 cases, 1 case was lost with skin and soft tissue expander exposing ;2 cases formated with hematoma, and cleaned the hematoma, expanding were contin- ued. All the other cases expanded and repaired with satisfactory. Conclusion According to different body area sear,choosing normal skin in- cision and soft tissue expander, preventing complications,reasonable design after skin expansion are points of successful operation.
