全玻璃体联合保留前囊膜晶状体切除治疗增生性糖尿病性视网膜病变疗效

摘　　要：目的探讨全玻璃体切除联合保留前囊膜晶状体切除术治疗增生性糖尿病性视网膜病变（PDR）的疗效。方法PDR患者30例（31只眼），均行保留前囊膜晶状体切除联合全玻璃体切除术，术中行超全视网膜光凝。其中8只眼行硅油充填，26只眼行II期人工晶体植入，术后随访10—15个月。结果26只眼（人工晶状体植入）术后视力显著提高，发生新生血管性青光眼1只眼，复发性视网膜脱离1只眼，视神经萎缩1只眼，3只眼前囊膜明显混浊。结论全玻璃体切除联合保留前囊膜晶状体切除治疗PDR是安全有效的方法。ObJective To observe the effect of total vitrectomy combined with lensectomy and reserving anterior capsule for proliferative diabetic retinopathy. Methods Total of 30 patients （31 eyes） who were suffered from proliferative diabetic retinopathy was taken treatment. All eyes accepted total vitrectomy combined with lensectomy reserving anterior capsule and panretinal photocoagulation. Silicone oil tamponade was done in 8 eyes, 26 eyes were implanted intraocular lens during two-stage operation. The follow-up was 10-15 months. Results Visual acuity was improved in 26 eyes after being implanted intraocular lens. One eye suffered from neovascular glaucoma. One eye suffered from recurrent retinal detachment. One eye suffered from optic atrophy. Severe opacification of anterior capsule occurred in 3 eyes. Conclusions Total vitrectomy combined with lensectomy and reserving anterior capsule is effective therapy for proliferative diabetic retinopathy.
