卡泊三醇联合丙酸氯倍他索软膏治疗寻常性银屑病的疗效观察

摘　　要：目的探讨卡泊三醇软膏联合丙酸氯倍他索软膏外用治疗寻常性银屑病的疗效及不良反应。方法采用随机、开放、平行自身对照试验。197例斑块状银屑病患者皮损沿躯干中线分为左右两侧，随机数字法选择其中一侧为治疗组，给予卡泊三醇软膏，涂皮损每天2次，丙酸氯倍他索软膏，涂皮损每天1次，4周为1个疗程；另一侧为对照组，给予复方丙酸氯倍他索软膏，涂皮损每天2次，疗程同治疗组。治疗前及治疗后每2周评价疗效，疗程结束时达到临床缓解的患者3个月后再次随访评价疗效1次。结果共有173例患者完成试验，治疗组治疗后2周和4周时症状积分下降指数（SSRI）、痊愈率及总有效率均显著高于对照组（均P〈0．05）。结论卡泊三醇软膏联合丙酸氯倍他索软膏外用治疗寻常性银屑病疗效确切，不良反应轻微。Objective To evaluate the efficacy and safety of calcipotriol ointment plus clobetasol propionate cream for the treatment of psoriasis vulgaris. Methods A randomized, open-label and self- controlled trial was performed. Totally, 197 patients with chronic plaque-type psoriasis were enrolled in this study. Psoriatic lesions in every patients were classified into the right and left parts along the median line of trunk, which were then assigned into the treatment group treated with topical 0.05% calcipotriol ointment twice daily and 0.02% clobetasol propionate cream once daily, and the control group treated with compound clobetasol propionate cream （containing 0.05% clobetasol propionate and 0.025% tretinoin） twice daily, by using a random number table. All the patients were treated for 4 weeks. Symptom score reducing index （SSRI） was calculated at the baseline and every week during the treatment for all the patients, and at 3 months after the end of the treatment for those who were relieved at the end of the treatment. Results Totally, 173 patients completed the trial. SSRI was significantly higher in the treatment group than in the control group on day 14 and 21 after the beginning of therapy （0.76 ＋ 0.17 vs. 0.47 ＋ 0.14, 0.90 ~ 0.21 vs. 0.59 ＋ 0.19,respectively, both P 〈 0.05）. Additionally, the treatment group showed an increased cure rate （75.72% vs. 45.67%, 82.66% vs. 54.91%, respectively, both P 〈 0.05） and total response rate （93.06% vs. 75.15%, 98.85% vs. 77.45% respectively, both P 〈 0.05） on day 14 and 21 compared with the control group. Conclusion Topical application of 0.05% calcipotriol ointment combined with 0.02% clobetasol propionate cream is indeed effective for the treatment of psoriasis vulgaris with mild adverse effect.
