摘 要:目的:探讨天津市汉族老年人群内皮型一氧化氮合酶(eNOS)基因第7外显子894位点和12内含子rs1800780位点多态性与原发性高血压(EH)的关系。方法:采用聚合酶链反应-限制性内切酶片段长度多态分析法(PCR-RFLP)对253例EH患者(EH组)和156例健康者(对照组)eNOS基因894位点及rs1800780位点进行基因多态性分型;同时检测所有研究对象的血脂等危险因素;分析不同基因型与EH发病的关系。结果:EH组年龄、BMI、TC及LDL-C与对照组相比均差异有统计学意义(均P<0.05);两组894位点基因型及等位基因频率分布差异无统计学意义;EH组rs1800780位点AG+GG基因型及G等位基因频率显著高于对照组(P<0.05);rs1800780位点AG+GG与高血压的关联差异有统计学意义(OR=1.956,95%CI:1.194~3.205,P=0.008);Logistic分析显示,影响高血压的危险因素有rs1800780G等位基因、年龄、BMI及LDL-C。结论:eNOS基因894位点多态性与EH无关,rs1800780位点携带G等位基因即基因型AG+GG与高血压有关,影响高血压的危险因素有rs1800780G等位基因、年龄、BMI及LDL-C。Objective:To investigate the relationship between exon 7th 894 site and intron 12th rs1800780 site polymorphism of endothelial nitric oxide synthase (eNOS) gene and essential hypertension (EH) in elderly Han people living in Tianjin. Method: The polymerase chain reaction-restriction fragment length polymorphism (PCR RFLP) method was used to detect genetypes of 894 site and rs1800780 site in 253 patients with EH (EH group) and 156 controls (control group). The plasma lipid levels and other risk factors were determined in all subjects. The relationship between genotypes and pathogenesis of EH was analyzed. Result:There were significant differ ences in age, BMI, TC and I.DL-C between EH group and control group (P(0.05). There were no significant differences in frequencies of allele and genotype distribution in 894 site of eNOS gene between two groups. The frequencies of G allele and AG+GG genotypes were significantly higher in EH group than those in control group (P〈0.05). The association between AG+GG genotype and EH was statistically significant (OR=1.956, 95% CI: 1. 194 3. 205, P=0.008). The independent risk factors of EH were G allele of rs1800780 site, age, BMI and LDL-C. Conclusion:The polymorphism of eNOS 894 site is not associated with hypertension, while the poly morphism of rs1800780 site is associated with hypertension. The independent risk factors of EH are G allele of rs1800780 site, age, BMI and LDL-C.
