基于聚类分析的施杞教授治疗腰椎间盘突出症的用药规律研究

摘　　要：目的:探讨和总结施杞教授治疗腰椎间盘突出症的学术思想和临床用药规律,探索名老中医经验整理的方法。方法:收集施杞教授治疗腰椎间盘突出症典型医案378份,对所选医案中符合纳入标准的药物运用Excel表进行收录,然后用SPSS 16.0for windows统计软件进行聚类分析。结果:①共有272份医案纳入研究,共计就诊428次;共得药物173味,出现的总频次为9623,常用中药有白芍、柴胡、川芎、当归等27味。②聚类分析发现益气活血药、补肾助阳药、祛风解表药、利水渗湿药、化痰通络药和健脾理气和胃药为施杞教授治疗腰椎间盘突出症的常用配伍药类。③圣愈汤、身痛逐瘀汤、独活寄生汤、苓桂术甘汤等为施老治疗腰椎间盘突出症的基本方药。结论:施老治疗腰椎间盘突出症体现出益气活血,兼顾痰瘀,重视补肾阴、肾阳的学术思想,聚类分析结果客观反映了施杞教授临床用药规律。To analyze the experience of professor Shi Qi in using medicine for treating lumber intervertebral disc herniation （LDH）, and to explore the method for summarizing the experience of famous practitioner of TCM. Methods： All 378 typical cases of LDH treated with professor Shi were collected. The herbs meeting the inclusion criteria in the selected literature were recorded by using Excel table, and analyzed by using SPSS 16.0 for windows statistical software. Results： 1. There are 272 medical cases were included in the study, for a total of 428 visits, involved 173 herb medicine, and total ap- pearance frequency was 9623. There were 27 kind drugs commonly used, such as Radix Paeoniae Alba, Bupleurum, Szech- wan Lovage Rhizome and Angelica, etc. 2. Cluster analysis found that the medicine commonly used by professor Shi Qi in the treatment of LDH were Yiqihuoxue decoction, Bushen Zhuyang decoction, Qufeng Shenshi decoction, Huatan Tongluo decoction, and Jianpi Liqihewei decoction. 3. The basic prescriptions for LDH used by professor SHI were Sheng-yu decoc tion, Shentong Zhuyu decoction, Duhuo Jisheng decoction, and Linggui Zhugan decoction. Conclusion： Supplementing qi and activating blood circulation, removing phlegm and blood stagnation, replenishing vital essence to tonify the kidney are the treatments for LDH by professor SHI. The clustering analysis results show objectively the rule of Professor Shi Qi in using medicine for treating LDH.
