摘  要:目的探讨肥厚性心肌(HCM)病患者左心室质量的相关因子。方法对22例HCM和20名正常志愿者行心脏电影MR及延迟增强扫描,以Segment软件计算左心室功能参数及延迟强化参数,对左心室质量指数与其他参数行单元及多元回归分析。结果 HCM患者的左心室质量指数远大于正常人(P均<0.001)。多元回归分析结果显示,HCM患者的最大室壁厚度与左心室质量指数相关(β=7.53,P=0.02);在收缩功能保持的HCM患者,左心室质量指数与左心室舒张末期容积指数(β=3.68,P=0.004)及最大室壁厚度(β=5.16,P=0.008)相关。结论最大室壁厚度是预测HCM患者各期别左心室质量的有效参数。对于收缩功能保持的HCM患者,估测其左心室质量的最佳参数为左心室舒张末期容积指数和最大室壁厚度。Objective: To explore the predict factors of left ventricular mass (LVM) in hypertrophic cardiomyopathy (HCM). Methods: Twenty-two patients with HCM and 20 normal volunteers underwent cardiac cine MR and delayed enhancement, LV functional parameters and delayed enhancement parameters were analyzed by Segment software. The functional parameters were indexed by body surface area. univariate and multivariate regression analysis were performed between LVM index and other parameters. Results: The LVM index in HCM patients significantly exceeded that of control subjects (P〈0.001). On multivariate analysis, only maximum left ventricular wall thickness (β=7.53, P=0.02) was associated with LVM index. In HCM patients with preserved systolic function, LV end-diastolic volume index (β=3.68, P=0.004) and maximal LV wall thickness (β=5.16, P=0.008) were correlated with LVM index. Conclusion: In patients with HCM, maximum LV wall thickness has useful value to predict LVM. The optimum parameters to estimate LVM in HCM patients with preserved systolic function are LV end-diastolic volume index and maximal LV wall thickness.
