双锁定钢板内固定与“Y”型钢板内固定治疗肱骨远端“C”型骨折的疗效比较

摘　　要：[目的]分析比较双锁定钢板内固定与"Y"型钢板内固定治疗肱骨远端"C"型骨折的临床效果,为临床上治疗肱骨远端"C"型骨折提供科学依据。[方法]2008年1月～2010年6月本院骨科就诊的肱骨远端"C"型骨折患者64例随机分为观察组和对照组,每组各32例。观察组接受双锁定钢板内固定法进行治疗,而对照组患者则接受"Y"型钢板内固定法进行治疗。比较两组患者的手术时间及术中失血量、骨折愈合时间、肘关节功能恢复情况及并发症情况。[结果]治疗组和对照组手术时间、手术中失血及骨折愈合时间差异无统计学意义(P>0.05),治疗组肘关节功能恢复优良率显著高于对照组(P<0.05),两组术后并发症情况差异无统计学意义(P>0.05)。[结论]双锁定钢板内固定和"Y"型钢板内固定治疗肱骨远端"C"型骨折在手术时间、术中出血、骨折愈合时间以及术后并发症方面相当,但是双锁定钢板内固定治疗术后肘关节功能的恢复更佳,值得临床推广。[ Objective]To analyze the clinical efficacy of double plate internal fixation and Y-shaped plate to treat distal humerus facture of C mode and to provide scientific basis to treat distal humerus facture of C mode. [ Method] The patients with distal humerus facture of C mode,who visited the orthopedics department of our hospital in January 2008-June 2010, were ran- domly divided into Observation Group and Control Group. There were 32 cases in each group. Cases in Observation Group received surgery of double plate internal fixation, and cases in Control Group received surgery of Y-shaped plate internal fixation. The duration of surgery, blood loss in surgery, time of facture healing,recovery of articulation cubital function and complications were compared in both groups. [ Result] The duration of surgery, blood loss in surgery, time of facture healing in both groups were no significantly different in statistics （ P 〉 0.05 ）. The recovered articulation cubital function in Observation Group was much better than that in Control Group （P 〈 0.05 ）. Complications in both groups were significantly different in statistics （P 〉 0.05）. [ Conclusion] Double plate internal fixation and Y-shaped plate internal fixation have appropriate effect to the duration of surgery, blood loss in surgery, time of facture healing and complications, and double plate internal fixation has better effect for recovered articulation cubital function. It is to be worthy being recommended in clinic.
