五苓散加减治疗膀胱过度活动症临床研究

摘　　要：目的:观察五苓散治疗膀胱过度活动症患者的疗效。方法:单中心的随机对照临床研究,将36例患者随机分成中药组24例和对照组12例,中药组以五苓散为基础方随症加减,对照组予舍尼亭口服,均治疗4周。观察治疗前后排尿日记、膀胱过度活动症症状评分表(OABSS)评分、国际前列腺症状生活质量评分(IPSS-QOL)、患者感知膀胱症状情况分级量表(PPBC)评分变化。结果:经4周的治疗后,2组的排尿次数、夜尿次数、尿急次数、尿失禁次数均有不同程度的减少,排尿次数及尿急次数治疗前后比较,差异均有显著性意义(P<0.05)。2组间比较,差异无显著性意义(P>0.05)。经4周治疗后,2组调查问卷中各项评分均有不同程度的下降,除尿失禁评分外,差异均有显著性意义(P<0.05)。2组间比较,差异无显著性意义(P>0.05)。中药组无不良反应。对照组有1例服药后出现口干,1例出现视物模糊,舍尼亭改为每天1次后均缓解。结论:五苓散和舍尼亭均能有效改善膀胱过度活动症患者的临床症状和生活质量。Objective： To evaluate the clinical effects of Wuling Powder for the treatment of overactive bladder. Methods： A monocentric randomized and controlled clinical trial was carried out in 36 overactive bladder patients. The patients were randomized into Chinese medicine group（n=24） and control group （n=12）, which were treated orally with modified Wuling Powder and Tolterodine Tartrate Tablets for 4 weeks, respectively. Daily urination, and the scores of Overactive Bladder Symptoms Scale（OABSS）, In- ternational Prostatitis Symptoms-Quality of Life（IPSS-QOL）, Patients Perception of Bladder Condition（PP- BC） were observed before and after treatment. Results： After treatment for 4 weeks, the frequency of urination, nocturia and urinary incontinence was decreased to various degrees in both groups, and the differences were significant compared with those before treatment（P〈 0.05） but were insignificant between the two groups（P〉0.05）. The scores of OABSS, IPSS-QOL and PPBC in both groups were decreased （P〈 0.05 compared with those before treatment）, but the differences between the two groups were in- significant（P 〉 0.05）. No adverse reaction was found in Chinese medicine group, but dry mouth and blurred vision respectively occurred in one case of the control group, and then the adverse reaction was relieved after the administration of Tolterodine Tartrate Tablets was adjusted to once per day. Conclusion： Both Wuling Powder and Tolterodine Tartrate Tablets can effectively relieve the symptoms and improve the quality of life of overactive bladder patients.
