摘  要:目的:探讨免疫细胞化学(ICC,immunocytochemistry)方法诊断胸腹水中细胞的良恶性和鉴别诊断恶性肿瘤细胞的组织学类型的临床意义。方法:对临床送检的胸腹水标本,除进行HE染色常规细胞学涂片和液基薄层细胞片(TCT)检查外,还将胸腹水制成石蜡包埋细胞块(CB,cell block),进行石蜡包埋细胞块的免疫细胞化学染色(CB-ICC),诊断细胞的良恶性,确定肿瘤细胞的组织起源。结果:186例患者,胸腹水细胞经免疫细胞化学鉴别诊断出:良性病变52例;恶性肿瘤134例,其中乳腺癌24例,肺癌27例,甲状腺癌18例,胃肠道癌22例,卵巢癌19例,肝癌9例,前列腺癌7例和间皮肉瘤8例,恶性肿瘤均经组织病理学证实。结论:病理细胞块的免疫细胞化学确定胸腹水中细胞的良恶性和鉴别诊断恶性肿瘤细胞的组织起源特异性强、敏感性高,具有客观、准确的特点,有着广泛的临床应用价值和推广前景。Objective: To investigate the feasibility of immunocytochemistry(ICC) in discriminate cells'origin and their nature from hydrothorax or aseites species. Methods: Hematoxylin and eosin(HE) stain was used as control for routine cellular smear and thin-cytologic test (TCT) staining, paraffin embedding was. used to prepare cell block, ICC was used to detect the cells' origin and their characteristics. Results:Among 186 cases, benign and malignant cancers diagnosed by ICC were 52 and 134, respectively. Of the malignant ones, cases of breast carcinoma, lung cancer, thyroid cancer, gastrointestinal tract cancer, ovarian cancer, hepatocarcinoma, prostatic cancer and mesotheliosarcoma was diagnosed as 24,27,18,22,19,9,7 and 8, respectively. All the malignant cancers were validated by histopathology. Conclusion: ICC conducted in histopathology is of high specificity and sensitivity in discriminating the cells'characteristic in hydrothorax or ascites species, thus should be proposed for general applieatiorL
