摘  要:目的:探讨斑点追踪技术测量二尖瓣环位移(Mitral annular displacement,MAD)评估右心功能不全对左室舒缩功能的影响。方法:研究对象为肺心病患者及健康人,选肺心病右心衰患者为病例组,选健康志愿者为对照组。在心尖四腔切面获取二尖瓣血流频谱E、A峰,并采集二维图像并将其输入QLAB6.0工作站获取左室侧壁二尖瓣环收缩期峰值位移(T1),室间隔二尖瓣环收缩期峰值位移(T2)。分析两组二尖瓣血流频谱及瓣环峰值位移的差别。结果:病例组E/A显著小于对照组(P<0.01);病例组T2小于对照组(P<0.05);两组间T1无显著性差异(P>0.05)。结论:右心功能不全时对左室的主要影响是降低了左室舒张功能及室间隔纵向收缩能力。Objective: To explore the effect of right ventricular dysfunction on left ventricular diastolic and systolic function using Mitral annular displacement (MAD) measured by speckle tracking technology.Methods: The subjects were pulmonary heart disease patients and healthy people.Patients were in the case group and the second group consisted of healthy volunteers for the control group.The procedure included viewing mitral inflow E/A peak in the apical four-chamber, acquiring two-dimensional images and getting them into QLAB6.0 workstation and finally getting mitral annular peak systolic displacement of left ventricular lateral (T1) and interventricular septum (T2).The differences in mitral inflow and annulus peak displacement were analyzed.Results: The case group E/A is less than the control group significantly(P<0.01) and so is T2(P<0.05), but there is no significant difference(P>0.05) in T1 between the two groups.Conclusion: The effect of right ventricular dysfunction on left ventricular is reducing left ventricular diastolic function and ventricular longitudinal contractility.
