摘 要:目的了解南京及周边地区因呼吸道疾病就诊患者血清抗嗜肺军团菌(Lp)1~14型抗体水平,分析抗Lp抗体阳性意义。方法采用德国Euroimmun间接免疫荧光(IFA)试剂对1 017例在南京军区南京总医院就诊患者血清标本进行抗Lp1~14型抗体效价测定。结果 1 017例患者血清抗Lp抗体阳性157例(15.4%)。Lp 14个血清型中,抗14型抗体检出率最高,为36.9%;其次为12型(32.5%)、4型(30.6%)。单独一种血清型抗体阳性者占54.1%(85/157);≥2种血清型抗体阳性者占45.9%(72/157)。在157例抗Lp抗体阳性患者中,抗EBV抗体阳性103例(65.6%),抗肺炎支原体(Mp)抗体阳性89例(56.7%),抗TB抗体阳性31例(19.7%)。结论目前使用的诊断Lp感染的IFA试剂,检测结果不能排除高假阳性率可能。诊断Lp感染必须遵循恢复期抗体效价较急性期升高≥4倍的原则。Objective To investigate the distribution characteristics of Legionella pneumophila (Lp) serotype 1 to 14 in the patients with respiratory tract infection in Nanjing and the surrounding areas and analyze the significance of positive results. Method Utilizing indirect immunofluorescence assay (IFA) (German, Euroimmun) to detect the antibody titer of Lpl to Lpl4 in ! 017 serum samples. Results Among the 1 017 samples, 157 ( 15.4% ) were Lp- positive. The serotype Lpl4 showed the highest positive rate (36.9%), followed by Lpl2 (32.5%) and Lp4 (30.6%). The predominant distribution pattern of Lp antibody was single serotype positive (54.1%, 85/157), and two or more than two positive serotypes were 45.9% (72/157). The positive rate of antibodies for tuberculo- sis, mycoplasma pneumoniae and Epstein Barr virus were 19.7%, 56.7%, 65.6%, respectively. Conclusion The current IFA di- agnostic reagent for Lp can not avoid the existence of highly false positive rates. The diagnosis of Lp infection should follow the princi- ple that the antibody titer of recovery phase should at least have a four-fold increase compared with the acute phase.
