1098例重复人工流产原因及需求分析

摘　　要：目的了解重复人工流产女性本次计划外妊娠的原因及生殖健康需求,为制定相应干预措施提供依据。方法对2005年9月～2006年8月期间,在北京市海淀区5所医院对因计划外妊娠实施早孕人工流产的女性进行问卷调查,了解此次计划外妊娠、人工流产的原因及其对生殖健康的需求。结果本次计划外妊娠的原因,44.0%(483/1098)为"避孕失败",20.5%(225/1098)为"没有坚持每次同房均采取避孕措施",35.5%(390/1098)的女性根本没有采取避孕措施。避孕失败者中,38.1%不知道避孕失败的原因;未避孕者及"未坚持每次同房均采取避孕措施"的首位原因均为"侥幸心理",分别占51.3%(200/390)、59.1%(133/225)。未婚者性伴侣不关心避孕的比例明显高于已婚者(χ2=38.244,P﹤0.001)。已婚女性终止此次妊娠的主要原因为"已有子女"(70.9%)。被调查者最希望通过医务人员了解避孕节育知识。结论医疗机构应加强人工流产后计划生育服务,增加有效避孕的可能性,减少计划外妊娠。OBJECTIVE To understand the reason and reproductive health needs of women with recurrent induced abortion, so as to provide basis for relevant intervention. METHODS 1 098 women who seeked induced abortion in five hospitals were investigated with self-administered questionnaire on the reasons of abortion and reproductive need, from September, 2005 to August, 2006 in Haidian District. RESULTS As for the cause of unplanned pregnancy, contraceptive failure accounted for 44.0% （483/1 098）, not always taking contraceptive measure accounted for 20.5% （225/1 098） , non-contraception accounted for 35.5% （390/1 098）. For contraceptive failure, 38.1% did not know detail reason. ＂Lucky thought＂ was the main reason for non-contraceptive （51.3%, 200/390） and not always taking contraceptive （59.1%, 133/225）. The proportion of unmarried sex partner who did not care about contraceptive is obviously higher than that of married couple （X^2 = 38.244, P 〈 0.001）. ＂Have had a child＂ is the main reason for abortion for married women （70.9%）. Medical workers were regarded as the best approach to acquire contraceptive knowledge. CONCLUSION Post-abortion family planning care should be improved to enhance effective contraceptive and to reduce unplanned pregnancy.
