摘 要:目的探讨超声心动图与脑利钠肽(brain natriuretic peptide,BNP)在川崎病(kawasaki Disease,KD)中的临床诊断价值,为川崎病不同时期采用不同的检查方法进行诊断提供科学依据。方法选取川崎病患儿35例为川崎病组,以发热、皮疹为主诉的肺炎支原体或病毒感染的48例患儿为对照组,用超声心动图检测急性期与恢复期左、右冠状动脉内径,同时,检测血中脑利钠肽水平,并做相关性分析。结果川崎病患儿恢复期冠状动脉内径值明显升高,高于对照组,差异具有统计学意义(P<0.05)。川崎病患儿急性期脑利钠肽高于对照组,差异具有统计学意义(43.9 vs15.8,P<0.01)。恢复期川崎病患儿冠状动脉扩张比例显著高于急性期KD患儿(P<0.05);川崎病患儿急性期脑利钠肽水平与恢复期左冠状动脉内径(LCAID)、右冠状动脉内径(RCAID)呈正相关(P<0.05)。结论超声心动图诊断川崎病及冠状动脉扩张在恢复期更有临床意义;脑利钠肽可作为急性期诊断川崎病的生化指标之一;急性期脑利钠肽值明显升高应警惕患儿恢复期冠状动脉扩张。Objective To investigate the significance of echocaradiography and brain natrinretic peptide (BNP, brain natriuretic peptide) in the diagnosis of the kawasaki disease, and to provide scientific basis for the diagnosis. Methods 35 patients with kawasaki disease for case group were enrolled, other 48 patients with mycoplasma pneumoniae or virus infection that complaint of fever and rash were enrolled as controlled group. We measured the left, right coronary artery diameter value and brain natriuretic peptide value in acute and recovery phase respectively, and did correlation analysis. Resolts In the reeovely phase, the eoronary artery diameter of children with kawasaki disease increased significantly, and there was statistical signifieanee; In the acute phase, brain natrinretie peptide of case group increased significantly, and there was statistical significance. The coronary artery expansion ratio in recovery phase was obviously higher than that of the acute phase of Kawasaki patients. Brain natriuretie peptide levels in the acute phase of kawasaki disease, and eonvaleseenee left coronary artery diameter (LCAID), fight coronary artery diameter (RLCAID) were positively correlated. Conclusions Eehoeardiography in diagnosis of kawasaki disease and eoronary expansion had more elinieal signifieanee in convalescence, and brain natrinretie pepticle can be used as one of the bioehemieal markers in diagnosis of kawasaki disease in acute phase. We should be alert of coronary artery expansion in recovery phase with patients who had higher brain natriuretie peptide in acute phase.
