低氧高二氧化碳环境下肺动脉内皮结构变化与肺动脉高压的关系

摘  要:探讨肺动脉内皮结构变化与低O2 高CO2 肺动脉高压的关系。SD大鼠分为对照组 (A组 ) ,4周低O2 高CO2 组 (B组 ) ,4周低O2 高CO2 +川芎嗪组 (C组 )。 1 电镜下B组肺细小动脉内皮细胞 (EC)向管腔内突起 ,呈立方或柱状上皮 ,部分细胞膜突起似绒毛 ,可见严重EC胞内水肿、变性 ,形成空泡 ,并可见EC下水肿 ,中膜平滑肌细胞增生 ,外膜胶原纤维高度密集 ,呈束状 ,C组肺细小动脉内皮细胞基本恢复正常 ,平滑肌细胞、胶原纤维增生明显减轻。 2 B组肺动脉平均压 (mPAP)明显高于A组 (P <0 .0 1) ,C组mPAP明显低于B组 (P <0 .0 1) ;3 光镜下 ,肺细小动脉管壁面积 /管总面积比值 (WA/TA)、肺细小动脉中膜平滑肌细胞核密度(SMC)B组较A组明显增高 (P均 <0 .0 1) ,C组较B组明显降低 (P均 <0 .0 1)。肺动脉内皮细胞严重水肿、变性 ,空泡形成与慢性低O2 高CO2 性肺动脉高压的形成密切相关 。To investigate relationship between structural changes of pulmonary arterial endothelium and chronic hypoxic hypercapnic pulmonary hypertension.Methods Thirty rats were randomly divided into three groups:control group (A),hypoxic hypercapnic group (B),hypoxic hypercapnia+ligustrazine (lig)group (C).Result (1) Electron microscopy showed notable intracellular edema of endothelium and subendothelial edema took place,the proliferation of smooth muscle cells and collageous fibers occured in pulmonary arterioles of chronic hypoxic hypercapnic rats,ligustrazine could invert the changes mentioned above.(2) mPAP was significantly higher in rats of group B than that of group A ( P <0.01) and group C ( P <0.01);(3) Light microscopy showed WA/TA (vessel wall area/total area) and SMC (the density of medial smooth muscle cells) were significantly higher in rats of group B than that of group A ( P <0.01) and group C ( P <0.01).Conclusion The impairment of endothelium in pulmonary arterioles has close relation with pulmonary hypertension induced by chronic hypoxic hypercapnia.
