经尿道输尿管镜治疗慢性精囊炎的临床研究

摘　　要：目的评价经尿道输尿管镜治疗慢性精囊炎及其伴发疾病的临床疗效。方法18例慢性精囊炎患者，单侧15例，双侧3例。18例均伴有会阴部不适，11例伴有血精，9例伴有前列腺炎，6例合并精囊结石。分别利用EMS第四代碎石清石系统及插管冲洗予以治疗，观察其临床疗效。结果手术时间25～55（31．6±8．6）min，住院时间5～10（5．2±1．8）d，术后随访3～6个月。3例进镜失败维持药物治疗；9例血精治愈，1例好转；15例会阴部不适好转；7例前列腺炎治愈，1例好转；5例精囊结石治愈，1例残留；2例术后3d及3个月时出现单侧附睾睾丸炎，给予抗炎对症治疗缓解，无直肠损伤等并发症。结论经尿道输尿管镜治疗慢性精囊炎及伴发疾病效果理想，尤其联合EMS第四代碎石清石系统，有较好的临床应用价值。Objective To evaluate the clinical effect of transurethral ureteroscopy for the treatment of chronic seminal vesiculitis and concomitant diseases. Methods Eighteen patients with chronic seminal vesiculitis, 15 patients were unilateral, 3 patients were bilateral. Eighteen patients with genital discomfort, 11 patients accompanied with hemospermia, 9 patients accompanied with prostatitis, 6 patients accompanied with seminal vesicle stones. Eighteen patients were treated by EMS fourth generation lithotripsy system and intubation douche. Results The operation time was 25 - 55 （31.6± 8.6） min, hospitalization time was 5 - 10 （5.2 ±1.8） d. Postoperative followed up for 3 -6 months. Three patients of imported failure maintained drug therapy, 9 patients of hemospermia were cured, 1 patient of them became better. Fifteen patients of genital discomfort became better. Seven patients of prostatitis were cured, 1 patient of them became better. Five patients with seminal vesicle stones were cured, 1 patient of them had residual. Two patients of postoperative 3 d and 3 months later had unilateral epididymo-orchitis. Symptomatic treatment was relieved with giving anti-i,flammatory. There were no complications. Conclusion Using EMS fourth generation lithotripsy system and intubation douche for the treatment of chronic seminal vesiculitis and concomitant disease is effective, and has better clinical application value.
