摘 要:目的探讨小儿头颅CT扫描参数变化对辐射剂量的影响及优化扫描参数的临床应用价值。方法选择我院疑诊头颅疾病需行CT扫描的不同年龄患儿300例,根据扫描参数管电压及管电流及螺距的不同分为以下3种情况:①A组管电压改变(80~120 kV),管电流及螺距不变;②B组管电流改变(100~300 mA),管电压及螺距不变;③C组螺距改变(0.8~1.5 mm),管电压及管电流不变。比较3种情况CT剂量加权指数(CTDIw)。结果 A组管电压80、100、120 kV时CT-DIw分别为(10.236±0.356)、(21.446±0.815)、(34.512±0.736)mGy;B组管电流100、200、300 mA时CTDIw分别为(12.293±0.226)、(23.446±0.915)、(35.842±0.736)mGy;C组螺距0.8、1.0、1.5 mm时CTDIw分别为(44.246±0.126)、(35.446±0.815)、(23.842±0.736)mGy。3种情况组内比较差异均有统计学意义(P<0.05)。结论在保证诊断质量的前提下,CT检查时适当降低管电压、管电流,提高螺距,可使辐射剂量明显降低。Objective To explore effect of parameter changes on radiation dose of head CT scans in children and clini- cal value of CT scanning parameter optimization. Methods A total of 300 patients of different ages with suspected cranial dis- eases, who would take CT scaning were divided into 3 groups by the tube voltage, tube current and pitch of scanning parameters : group A with the voltage change (80-120 kV), changeless pitch and tube current; group B with change of tube current (100- 300 mA), changeless tube voltage and pitch; group C with pitch change (0.8-1.5 ram) , changeless tube voltage and tube cur- rent. CT dose weighted index numbers (CTDIw) of the 3 groups were compared. Results The CTDIw were( 10.236 ± 0.356), (21.446±0.815) and (34. 512 ±0.736) mGy respectively in group A, when tube voltages were 80, 100 and 120 kV corre- spondingly ; the CTDIw were ( 12. 293 ± 0. 226) , (23. 446 ± O. 915 ) and ( 35. 842 ± 0. 736 ) mGy respectively in group B, when the tube currents were 100, 200 and 300 mA correspondingly; the CTDIw were (44. 246 ± 0. 126 ) , (35. 446 ± 0. 815 ) and (23. 842 ± 0. 736) mGy respectively in group C ; when the pitches were 0.8, 1.0 and 1.5 mm correspondingly, the differences of the three groups were statistically significant (P 〈 0. 05). Conclusion On the premise of ensuring the quality of diagnosis, ap- propriate reductions of tube voltage and tube current, and increase of pitch in head CT examination for children can significantly reduce radiation dose.
