摘  要:目的:探讨胃血流灌注在急性脑血管病引发胃黏膜应激损伤中的作用。方法:健康成年雄性Wistar大鼠,随机分为脑缺血再灌注(MCAO)组、脑缺血假手术(MCAO-sham)组、胃缺血再灌注(GI-R)组和胃缺血假手术(GI-R-sham)组。采用线栓法制作大鼠局灶性脑缺血再灌注模型;观察大鼠脑缺血再灌注胃黏膜与局部缺血胃黏膜组织病理学变化的异同。结果:MCAO组和GI-R组大鼠胃黏膜的病理改变相同,可见黏膜糜烂、点状出血;上皮细胞明显受损,有坏死、脱落;腺体排列紊乱;中性粒细胞、单核细胞等炎性细胞浸润等。免疫组织化学显色显示两种缺血模型大鼠胃黏膜有相似程度的胃泌素和生长抑素的表达变化。结论:胃黏膜血流灌注降低是大鼠脑缺血再灌注引发胃黏膜应激损伤的首要因素。Objective: To determine the effect of blood flow of the stomach on stress-induced gastric mucosal lesions caused by acute cerebrovascular disease. Methods: Forty adult male Wistar rats were randomly divided into 4 groups: middle cerehral artery occlusion (MCAO) and reperfusion group (MCAO group), MCAO sham-operated group (MCAO-sham group), gastric ischemia-reper{usion (GI-R) group, GI-R sham-operated group (GI-R-sham group). A focal cerebral ischemia and reperfusion rat model was established with left MCAO by using an intraluminal filament, followed by reperfusion. The histo- logical alterations of the gastric mucosa in different experimental conditions were observed. Results: Mucosal erosion, punc- tare hemorrhage, damage of epithelial cells, and infiltration of inflammatory cells was observed in either MCAO or GI-R rats. Immunohistochemical staining showed that the levels of gastrin (Gas) and somatostatin (SST) expression in MCAO rats were similar to those in GI-R rats. Conclusion: The decrease in blood flow of the stomach is the primary cause on gastric mucosal lesions after acute cerebrovascular disease.
