输尿管镜气压弹道碎石治疗输尿管中下段结石临床疗效观察

摘　　要：目的观察采用输尿管镜气压弹道碎石术治疗后,输尿管中下段结石患者的结石排空情况及术后并发症。方法来我院进行治疗的输尿管中下段结石患者101例,在患者同意的情况下,遵照随机原则分为两组。治疗组患者51例,用输尿管镜气压弹道碎石术进行治疗;对照组患者50例,采用体外冲击波碎石术治疗。比较两组患者的结石排净率及并发症情况。结果治疗组51例患者,全部碎石成功(100%),结石排空率为96.08%;对照组50例患者,36例碎石成功(72.00%),排空率为62.00%。两组患者手术碎石成功率比较,差异有统计学意义(P<0.05);两组患者1个月时结石排空率比较,差异有统计学意义(P<0.01)。治疗组主要存在的术后并发症包括术后感染、输尿管穿孔,其中术后感染1例,输尿管穿孔1例;对照组主要存在的并发症涉及术后感染、输尿管狭窄、再次手术,其中术后感染2例,输尿管狭窄7例,再次手术12例。结论对输尿管中下段结石的治疗,输尿管镜气压弹道碎石治疗应作为其首选。Objective To observed transureter0scopic pneumatic lithotripsy in the treatment of urethral stones in the middle and lower segments of the urethra. Methods 101 patients were randomly divided into a treatment group and a control group. 51 cases of patients treated with ransureter0scopic Pneumatic Lithotripsy; 50 patients of the control group, using extracorporeal shock wave lithotripsy for treatment. Contrast stone clearance rate of the two groups of patients, the incidence of postoperative infections and complications. Comparing two groups of patients stone free rate and complications. Results The treatment group of 51 patients, of all gravel success 000%）, the stone-free rate was 96.08%; 50 patients of the control group, 36 cases of the gravel successful （72.00%）, emptying rate is 62.00%. The two groups were compared surgery gravel success rate （x2 = 0. 14, P 〈0.05）, the difference was statistically significant; comparing two groups of patients stone-free rate （x2 = 0.02, P 〈0.01）, the difference was statistically significant, Conclusion For the treatment of lower urethral stones in the middle and lower segments of the urethra, transureter0scopic pneumatic lithotripsy as their first choice.
