摘 要:【目的】探讨超声实时弹性应变率比值在甲状腺良恶性结节鉴别诊断中的应用价值。【方法】回顾性分析本院行甲状腺占住手术患者105例(288个结节)的甲状腺超声图像,所有患者术前均行甲状腺超声弹性检查及测定弹性应变率比值,术后与病理检查结果对照分析,采用受试者工作特征(ROC)曲线确定弹性应变率比值的最佳临界值。【结果】甲状腺结节良性组的平均应变率比值为2.06,显著低于恶性组的7.83,其差异有统计学意义(Pd0.05);通过R0c曲线确定鉴别诊断甲状腺良恶性结节的弹性应变率比值为3.28,弹性应变率比值判断甲状腺结节良恶性的敏感性、特异性和准确性分别为85.7%(36/42)、95.9%(236/246)、94.4%(272/288)。【结论】超声弹性应变率比值可作为超声鉴别诊断甲状腺良恶性结节的一种客观指标,弹性应变率比值在甲状腺良恶性结节鉴别诊断中有应用价值。[Objective] To explore the value of real--time ultrasound elastic strain rate ratio in the differ- ential diagnosis of benign and malignant thyroid nodules. [Methods]The thyroid ultrasound images of 105 pa- tients(288 nodules) undergoing thyroid operation in our hospital were analyzed retrospectively. Ultrasound e- lastic examination of thyroid was performed and elastic strain rate ratio was measured in all patients before the operation. The results were contrasted with pathological results after the operation. The receiver operating characteristic(ROC) curve was used to determine the optimal threshold of elastic strain rate ratio. [Results] The average elastic strain rate ratio in benign thyroid nodule group was 2.06, which was markedly higher than that in malignant thyroid nodule group(7.83), and there was significant difference( P 〈0.05). The elastic strain rate ratio determined by ROC curve for the differential diagnosis of benign and malignant thyroid nodules was 3.28. The sensitivity, specificity, and accuracy of elastic strain rate ratio for the differential diagnosis of benign and malignant thyroid nodules were 85.70%(36/42), 95.9%(236/246) and 94.4%(272/288), respec- tively. [Conclusion] Ultrasound elastic strain rate ratio can be considered as a objective index for the differenti- al diagnosis of benign and malignant thyroid nodules. The elastic strain rate ratio is valuable for the differential diagnosis of benign and malignant thyroid nodules.
