摘  要:【目的】探讨血清胱抑素C(Cysc)、糖化血红蛋白(HbAlc)、超敏c反应蛋白(hs—CRP)等生化指标与2型糖尿病(T2DM)微血管病变的关系。【方法】选择本院内分泌科住院患者及门诊正常体检者265例,其中正常人(NC)65例,糖尿病合并微血管病变(DMA)者100例,糖尿病无微血管病变(NDMA)者100例,分别测定其CysC、HbAlc、hs—CRP及血脂等生化指标,并测定一般临床资料,记录年龄、病程、血压、体重指数等。【结果1T2DM患者外周血中空腹血糖(FBG)、餐后两小时血糖(2hPG)、低密度脂蛋白(LDL)、HbAlc、CysC、hs—CRP浓度较NC组显著增高(P〈0.05),且DMA组患者外周血2hPG、HbAlC、甘油三脂(TG)、CysC、hs—CRP浓度明显高于NDMA组(P〈0.05)。DMA与血清CysC、2hPG、HbAlC、hs—CRP、年龄、病程相关(P〈0.05)。多元Logistic回归分析显示仅年龄、病程、HbAlC、hs—CRP是DMA的危险因素。【结论】DMA的发生、发展除与年龄、病程等因素相关外,还与HbAlC、hs—CRP有关。[Objective] To explore the relationship of biochemical indicators Cystatjn C (CysC), glycosylated hemoglobin(HbAlc) and high sensitivity C-reactive protein(hs-CRP) with Type 2 diabetic microangiopathy (DMA). [Methods]Totally 100 cases of DMA, 100 cases of diabetes mellitus without microangiopathy(NDMA) and 65 normal controls(NC) in endocrinological department of our hospital were chosen. The biochemical indica- tors such as CysC, HbAlc, hs-CRP and blood lipids were measured. The common clinical data were analyzed. The age, course, blood pressure and body mass index were recorded. [Results] Fasting blood glucose(FBG), postprandial 2h blood glucose(2hPG), low density lipoprotein(LDL), HbAlc, Cys C and hs-CRP in peripheral blood of patients with type 2 diabetes mellitus were significantly higher than those in NC group( P 〈0.05). The 2hPG, HbAlc, triglyceride(TG), Cys C and hs-CRP in peripheral blood of DMA group were significantly higher than those of NDMA group(P^0.05). DMA was positively correlated with Cys C, 2hPG, HbAlc, hs-CRP, age and course( P〈.05). Multiple logistic regression analysis showed that only age, course, HbAlc and hs-CRP were the risk factors of DMA. [Conclusion] Not only the pathogenesis and development of DMA are related with the course and age, but also associated with HbAlc and hs-CRP.
