经后路椎间盘镜下置入腰椎间融合器治疗腰椎间盘突出症临床观察

摘　　要：目的观察经后路椎间盘镜下置入腰椎间融合器Cage治疗腰椎间盘突出症的效果。方法 2008年3月—2011年6月笔者采用后路显微椎间盘镜手术系统(METRx)完成髓核摘除术后,置入腰椎间融合器Cage治疗腰椎间盘突出症32例,观察临床效果。结果本组均随访1～4年,仅1例出现Cage轻度下沉,椎间隙变窄;3例出现轻度椎间隙变窄。随访期间未出现椎管狭窄、椎体滑脱、椎间盘突出复发及神经根受压症状。结论 METRx未改变镜下、微创的原则,软组织、骨组织损伤少,椎体的稳定性未改变,但由于病例少、随访时间短,远期效果有待验证。Objective To observe the effect of lumbar cage insertion under the posterior intervertebral disc mirror in treatment of lubar intervertebral disc protrusion. Methods The author performed lumbar cage insertion and nucleus pulposus extraction with the posterior microscopic intervertebral disc mirror operation system （METRx） during March 2008 and June 2011 in treatment of 32 patients with lubar intervertebral disc protrusion, and the effect was observed. Resuits All the patients were followed up for one to four years. There was 1 case of Cage slightly subsidence and 3 cases of mild intervertebral space narrowing. During the follow-up there was no spinal stenosis, basivertebral detachment, slipped disk recurrence and root compression symptoms. Conclusion The method does not change the mirror and mini- mally invasive principle which has little soft tissue and bone tissue damage, and the stability of the vertebral body has not changed. The long-term effect is difficult to be sure due to lesser cases and short follow-up time.
