单臂外固定器结合腓骨重建钢板治疗Ⅲ型胫腓骨远端开放性粉碎性骨折效果观察

摘　　要：目的观察单臂外固定器结合腓骨重建钢板治疗Ⅲ型胫腓骨远端开放性粉碎性骨折的效果。方法2002年1月—2010年1月笔者应用Orthofix单臂外固定器结合腓骨重建钢板治疗Ⅲ型胫腓骨远端开放粉碎性骨折38例,观察临床效果。结果本组随访时间平均29个月。其中创面感染4例,针道感染3例;骨折延迟愈合2例,骨不连1例,经治疗后痊愈。踝关节功能优良率为92.1%。结论单臂外固定器结合腓骨重建钢板治疗胫腓骨远端开放性粉碎性骨折具有操作简单、固定牢固、微创等优点,可有效地减少感染、骨不连等并发症,是一种比较安全、可靠的治疗方法。Objective To explore the effect of single-arm external fixator combined with reconstruction plate fixation in treatment of Gustilo Ⅲ open comminuted fractures of distal tibiofibula. Methods During January 2002 and January 2010 the author performed single-arm external fixator combined with reconstruction plate fixation in treatment of 38 patients with Gustilo Ⅲ open eomminuted fractures of distal tibiofibula. The clinical effects were analyzed. Results All cases were followed up for average 29 months, there were 4 cases of wound infection, 3 cases of pin hole infection, 2 cases of delay union and 1 case of bone nonunion. All the cases were cured, and fineness rate of malleolar joint function was 92.1%. Conclusion The method of single-arm external fixator combined with reconstruction plate fixation in treatment of Gustilo Ⅲ open comminuted fractures of distal tibiofibula is safe, reliable and microtrauma which can effectively reduce complications such as infection and bone nonunion.
