臀肌挛缩症的步态特征生物力学分析

摘　　要：目的为临床治疗与康复实施提供客观依据。方法对臀肌挛缩患者常速及快速行走时的步态数据进行采集与分析。结果快速行走时臀肌挛缩症患者的步态周期小于正常人(P=0.0389,P<0.05),步频大于正常人(P=0.0124,P<0.05)。患者在常速、快速行走时髋关节摆动相最大屈曲角度与正常人之间存在统计学差异(P=0.0049,P<0.05)、(P=0.0327,P<0.05);常速及快速行走时臀肌挛缩患者髋关节向严重侧与向非严重侧的旋转角度差均大于正常人(P=0.0230,P<0.05);快走时臀肌挛缩患者步态的支撑期膝关节屈曲角度大于正常人(P=0.03140,P<0.05)。臀肌挛缩患者在常速行走时重心晃动与正常人之间存在统计学差异(P=0.0370,P<0.05)。结论①臀肌挛缩患者行走时步频小于正常人,下肢双支撑时间所占比例大于正常人。②患者行走时髋关节过度后伸,髋关节屈曲受限,向左右两侧旋转不对称。Objective To Provide an objective basis for the treatment and rehabilitation of the patients with gluteal muscle contracture.Methods The walking gait data at normal and high speed in patients with gluteal muscle contracture were selected and analyzed.Results At the high speed walking,the gait cycle of patients with gluteal muscle contracture was less than that of normal subjects（P= 0.0389,P0.05）,but the stride frequency was greater（P= 0.0124,P0.05）.The maximum hip flexion angle in hip swing phase at normal and high speed walking were statistically significant difference（P=0.0049,P0.05;P=0.0327,P0.05）;The hip rotation angle to both serious side and non-serious side at normal and high speed walking in gluteus contracture patients were greater than those in normal people（P=0.03140,P0.05）;The knee flexion angle during the stance phase of gait at high speed in patients with gluteal muscle contracture was greater than that in normal people（P=0.03140,P0.05）;The differences in postural sway at normal speed walking was statistical significant between the patients with gluteal muscle contracture and normal people（P=0.0370,P0.05）.Conclusion The walking stride frequency of patients with gluteal muscle contracture is less than that of normal people,the support time of lower limbs is greater than that of normal people.The patients are usually with hip over-extension,hip flexion is limited,and rotation to the right and left is asymmetry.
