瑞芬太尼复合异丙酚在小儿等离子扁桃体腺样体切除术中的应用

摘　　要：目的探讨在小儿等离子扁桃体腺样体切除术中瑞芬太尼复合异丙酚对循环及全麻苏醒的影响。方法全麻下行小儿等离子扁桃体腺样体切除术60例,随机分为两组,瑞芬太尼复合异丙酚组(R组)和氯胺酮复合异丙酚组(K组)。观察两组患者诱导前、插管前、拔管后5min的HR、MAP、SpO2,以及全麻苏醒时间、拔管时间、躁动发生情况。结果 R组拔管后的HR、MAP明显低于K组(P<0.05),全麻苏醒时间、拔管时间、躁动发生情况明显少于K组(P<0.01)。结论瑞芬太尼复合异丙酚应用在小儿等离子扁桃体腺样体切除术中,患儿生命指征平稳、苏醒快、躁动发生率低。Objective To explore the effect of remifentanil combined propofol to circulation and recovery from general anesthesia in children plasma tonsillectomy and adenoidectomy.Methods Chose 60 pediatric patients who had plasma tonsillectomy and adenoidectomy with general anesthesia,divided into two groups randomly.Remifentanil combined propofol group（R group）,ketamine combined propofol group（K group）,observed their occurrence of agitation,time of tube drawing,time of recovery from general anesthesia,and their HR,MAP,SpO2 before induction of anesthesia,before intubation and 5 minutes later from tube drawing.Results HR,MAP of group R after drawing tube was lower significantly than that of group K（P〈0.05）,time of recovery from general anesthesia,and time of tube drawing,occurrence of agitation,group R was less than that of group K obviously（P〈0.01）.Conclusion Remifentanil combined propofol used in pediatric plasma tonsillectomy and adenoidectomy make children＇ vital signs placidly,recovery quickly,and have a low incidence of agitations.
