摘 要:目的探讨甲状腺疾病患者手术中实时喉上神经外侧支监测避免神经损伤的应用价值。方法 2011年2月~2012年11月90例甲状腺手术患者,术中使用气管套管式表面电极插管实施神经监测、定位和保护神经。手术前后通过嗓音障碍指数(VHI)量表10评价声音质量变化,喉镜检查观察声带活动情况。结果手术定位喉上神经外侧支124条,所有患者手术后均无发音嘶哑、音调降低表现,喉镜提示声带活动无明显受限,术前、术后VHI-10评分对比,差异无显著性(P>0.05)。结论甲状腺患者手术中喉上神经监测技术有利于定位神经,减少医源性损伤。Objective To evaluate the viability of intraoperative neuromonitoring of the external branch of the superior laryngeal nerve (EBSLN) during thyroidectomy to avoid nerve injury. Methods 90 patients with 124 external branch of the superior laryngeal nerves at risk were enrolled in this study from Feb 2011 to Nov 2012. NIM-Response electromyographie mon- itor system was used for operation. Laryngoscopy and voice handicap index(VHI)-10 were utilized pre and post operation. Re- sults All nerves were located successively and no patient had a significant voice alteration.There was no limitation of vocal cord movement in all patients. The VHI score level of pre and post operation had no significant difference (P〉0.05). Conclusion Intraoperative neuromonitoring is useful and helpful in avoiding EBSLN injury during thyroidectomy.
