亚高原高血压与糖尿病的关系

摘　　要：目的了解昆明地区城镇高血压与糖尿病的关系。方法单纯随机方法抽取18岁以上人群中血压异常符合高血压诊断标准及体检中血压正常两组人员作空腹血糖、胰岛素检测及HOMA-IR对比、分析。结果高血压组血糖异常人数明显高于正常对照组(χ2=194.4,P<0.05),高血压组空腹血糖浓度、胰岛素浓度也较对照组高,胰岛素抵抗较对照有明显增加(t=13、7.3、3.5,P<0.01)。结论地处亚高原的昆明地区城镇高血压发病与糖尿病有关,糖尿病的病因除胰岛素功能改变外,更主要表现为周围组织存在胰岛素抵抗,糖耐量异常也是高血压的危险因素。积极控制血压应从改变生活方式开始,药物治疗从控制血压、血糖及血脂三方面应同时进行,提高治疗效果并降低死亡率。糖尿病患者中密切注意高血压的早期诊断,积极治疗,减少及延缓并发症出现。Objective To understand the relationship of the Kunming area town of hypertension and diabetes. Methods Contrasted and analyzed the fasting glucose,insulin detection and HOMA-IR both groups with simple random method to extract the abnormal blood pressure in people over the age of 18 meet the diagnostic criteria of hypertension and physical examination with normal blood pressure. Results In hypertension group, the number of abnormal blood glu- cose and insulin resistance were significantly higher than the normal control group （~2=194.4,P 〈 0.05）,and （t = 13, 7.3,3.5,P 〈 0.01）,respectively. The fasting plasma glucose concentration and insulin concentration were higher than the control group, too.＇ Conclusion The Kunming area town located in the Sub-plateau,hypertension and diabetes re- lated to the etiology of diabetes, insulin function changes, mainly for the surrounding tissue to insulin resistance, im- paired glucose tolerance and hypertension risk factors. Actively control the blood pressure should change the lifestyle. Medication from the control of blood pressure, blood sugar and blood lipids in three areas should be carried out at the same time, to improve treatment and reduce mortality. Diabetic patients, pay close attention to the early diagnosis of hypertension, active treatment, and reduce and delay complications.
