摘 要:目的探讨代谢综合征(MS)在老年人良性前列腺增生(BPH)病程中的作用。方法 182例确诊BPH患者分为单纯BPH组98例与合并MS的BPH组84例,分析体质指数(BMI)、腰围、收缩压、空腹血糖(FBG)、高密度脂蛋白胆固醇(HDL-C)等代谢因素对BPH患者前列腺体积(PV)、国际前列腺症状评分(IPSS)、最大尿流率(Qmax)、前列腺体积年均增长率的影响。结果合并MS的BPH组患者表现为较高BMI、高血压、高血糖、高甘油三酯、高低密度脂蛋白和高IPSS评分、前列腺体积、前列腺年均增长率,但Qmax则明显低于BPH组(P<0.05)。Pearson’s相关分析显示前列腺体积与年龄、BMI、腰围、收缩压、空腹血糖、低密度脂蛋白呈正相关,与高密度脂蛋白呈负相关。IPSS评分与年龄、BMI、腰围、空腹血糖、总胆固醇、低密度脂蛋白呈正相关,与高密度脂蛋白呈负相关。Qmax与年龄、腰围、空腹血糖、总胆固醇及低密度脂蛋白呈负相关。前列腺年均增长率与年龄、BMI、收缩压、空腹血糖及低密度脂蛋白呈正相关。多元线性回归分析提示血糖与前列腺各指标关系最密切。结论 MS对BPH的病程及发展具有明显的影响。Objective To explore the effect of metabolic syndrome (MS) on the occurrence and development of benign prostatic hyperplasia (BPH). Methods All 182 elderly patients with diagnosed BPH were divided into two groups: BPH group (n = 98) and BPH with MS group (n = 84). The effects of metabolic indexs, including body mass index (BMI), waist, blood pressure, fasting blood glucose(FBG) and high density lipoprotein cholesterol (HDL-C), prostate volume (PV), international prostate symptom score(IPSS), maximum flow rate(Qmax) and the PV average annual growth rate were surveyed in BPH patients. Results BPH with MS group showed a higher body mass index, high blood pressure, high glycemia, high triglycerides, high low density lipoprotein cholesterol(LDL-C) and high IPSS, PV and its average annual growth rate, but a lower maximum flow rate(P 〈 0.05). The PV was positively correlated with age, BMI, waist, blood pressure, FBS, LDL-C and negatively correlated with HDL-C. The IPSS was positively correlated with age, BMI, waist, FBS, total cholesterol (TC), LDL-C and negatively correlated with HDL-C. The Qmax was negatively correlated with age, waist, FBS, TC and LDL-C. The PV average annual growth rate was positively correlated with age, BMI, blood pressure, FBS and LDL-C. Multiple linear regression analysis showed that the blood glucose was closely corre- lated with the indexs of prostatic hyperplasia. Conclusion MS has evident effects on the occurrence and the develop- ment of BPH.
