摘 要:目的 比较更年期妇女与健康育龄妇女阴道菌群构成及数量的差异.方法 采集10例健康育龄妇女,6例更年期妇女阴道分泌物标本,构建阴道细菌16SrDNA基因文库,对3 200个阳性克隆子进行测序鉴定.比较更年期和健康育龄期妇女阴道菌群的构成差异.采用荧光定量PCR方法比较两组标本中优势乳酸杆菌数量的差异.结果 健康妇女阴道菌群构成单一,多为2~3种细菌,乳酸杆菌属细菌占绝对优势,在更年期妇女和健康育龄期妇女分别为86.7%和99.2%.健康育龄期妇女阴道菌群构成以卷曲乳酸杆菌为主占72.2%,更年期妇女阴道菌群构成以惰性乳酸杆菌为主占81.4%.更年期妇女阴道乳酸杆菌的16SrDNA基因拷贝数对数值为5.27±0.31(Log10 copies/ml),较健康育龄期妇女5.76±0.35(Log10 copies/ml)明显减少(z=-2.242,P〈0.05),其中卷曲乳酸杆菌数量由育龄期的5.62±0.32(Log10 copies/ml)下降为更年期的3.74±0.42(Log10 copies/ml),差异具有统计学意义(z=-3.256,P〈0.05).两组标本中的惰性乳酸杆菌16SrDNA基因拷贝数差异无统计学意义(z=-0.543,P〉0.05).结论 更年期妇女和健康育龄期妇女阴道菌群构成上存在差异,乳酸杆菌的种类和数量发生了明显改变.Objective To confirm the vaginal flora characteristics of healthy menopausal women and compare the vaginal flora differences between menopausal age and child-bearing age. Methods Broad-range PCR amplification of 16SrDNA and construction of vaginal community library were used to analyze the vaginal flora diversity of 10 child-bearing women, 6 menopause women. Total 3 200 positive clones were sequenced and identified. Fluorescence quantitative PCR were used to compare the Quantity discrepancy in two groups. Results 2 to 3 vaginal bacterial species were found in healthy women vaginal. Vaginal lactobacilli were the predominant bacterium in healthy women vaginal. Lactobacillus in menopausal women and in child-bearing women was 86.7 % and 99.2 %, respectively, Lactobacillus erispatus was the predominant bacterium in childbearing women (72.2%). Lactobacillus iners was the predominant bacterium in menopausal women (81.4%). Compared with child-bearing women the number of vaginal lactobacillus 16SrDNA genes of menopausal women were decreased significantly,from 5.76±0. 35 (Logl0copies/ml) to 5.27±0.31 (Log10 copies/ml). The difference was significantly (z= - 0. 242,P〈0.05). The number of L. crispatus 16SrDNA genes decreased from 5.62± 0.32 (Log10copies/ml) to 3.74±0. 42 (Log10copies/ml) (z=- 3. 256, P〈0.05), No statistically significant differences in amount of L. Iners 16SrDNA genes were found among the two groups (z= -0. 543 ,P〉0.05). Conclusion The findings indicate further diversity of bac- terial species in the women vagina in different physiology period. The species and quantity of laetobacillus in the women vagi- nal changed obviously.
