摘 要:目的 研究肺结核病人外周血T淋巴细胞上CD95与程序性死亡因子-1(PD-1)及其配体PD-L1的表达情况,初步探讨其在肺结核发生发展中的意义.方法 采用流式细胞术(Flow Cytometry,FCM)检测63例初治活动性肺结核患者外周血T淋巴细胞上CD95与PD-1,PD-L1的表达水平,并与正常人对照,进行临床分析.结果 初治活动性肺结核患者外周血T淋巴细胞上CD95的表达率为(62.07±14.87)%,与正常对照组(35.74±10.14)%比较显著升高(t=8.515,P<0.05);PD-L1的表达率为(26.55±12.8)%,与正常对照组(18.56±6.46)%比较显著升高(t=3.127,P<0.05);而PD-1的表达率(13.64±9.10)%与正常对照组(28.71±7.06)%比较显著降低(t=-7.766,P<0.05);经相关性分析,初治活动性肺结核患者外周血T淋巴细胞上CD95与PD-1的表达水平呈正相关,差异具有统计学意义(r=0.279,P<0.05),CD95与PD-L1表达水平无确定相关性(r=-0.025,P>0.05).结论 CD95和PD-L1在肺结核病患者T淋巴细胞上的表达明显升高,这可能在肺结核病的发生、发展过程中起重要作用.Objective To investigate the expression of CD95,programed death-1 (PD-1) and its ligands including PD-L1 on T-lymphocyes in the peripheral blood from pulmonary tuberculosis patients and the role of this expression in genesis and development of chronic tuberculosis pulmonary. Methods 63 initial treatment of active pulmonary tuberculosis patients were detected by Flow Cytometry for partition CD95 and PD-1, PD-L1 on peripheral blood T-lymphocyes, with 28 healthy persons as control group. Results In the initial treating active pulmonary tuberculosis diseas the expression rate of CD95 was (62.07 ±14. 87)%, compared with the control group (35. 74±10. 14)% were significantly increased (t= 8. 515,P〈0. 05) ;the expression rate PD-L1 was (26.55 ± 12.8)% ,compared with the control group (18. 56 ±6.46)% was significantly increased (t=3. 127 ,P〈0. 05) ;but the levels of PD-1 (13.64±9.10) % in the supernatant were significantly lower(t= -7. 766,P〈 0. 05) ,compared with the control group(28. 71±7.06)%. Through correlation analysis,the expression levels of CD95 ,PD-1 on the peripheral blood T-lymphocyes in patients with pulmonary tuberculosis were positively correlated,and the difference was statistically significant (r= 0. 279, P〈0.05), but the levels of CD95 and PD-L1 did not determine relevance (r= - 0. 025, P〉0. 05). Conclusion Presume that high CD95 and PD-L1 expression may play a role in the pathogenesis and development of tuberculosis pulmonary.
