正中神经缩窄术治疗腕一指屈曲痉挛患者的护理

摘　　要：目的探讨正中神经缩窄术治疗腕一指屈曲痉挛的脑瘫患者的护理。方法对收治的20例实施正中神经缩窄术治疗的腕一指屈曲痉挛的脑瘫患者给予特殊的护理及康复训练指导，采用改良Asworth评分量表评估患者的肌张力，采用简化Fud-Meyer运动功能评分法评估患者上肢运功功能。结果20例患者随访18～24个月，平均22个月。术后近期痉挛缓解率为96．4％，随访时为95．2％。术后近期运动功能改善率为83．6％，随访期间为92．8％。所有患者未出现伤口不愈合、肌无力等并发症。结论正中神经缩窄术可以有效缓解腕一指痉挛状态，全程良好的护理和康复指导是手术成功和功能恢复的有效保证。Objective To explore the nursing instructions for spastic cerebral palsy patients accepted selective median neurotomy on spasticity focalized to wrist and finger. Methods Totals of 20 patients who were treated for spasticity of wrist and finger by selective median neurotomy were carried out specialized nursing and rehabilitation guidance. The effects of nursing and rehabilitation were evaluated by the scale of improved Asworth rating and Simplified Fugl-Meyer movement function scoring method. Results All patients obtained the good nursing and rehabilitation guidance before and after operation. The spasticity relieved obviously （95.2% vs 96.4% ） and the motor function improved significantly on affected limb （92. 8% vs 83. 6% ） after 18-22 months of operation. No complication occurred. Conclusions Selective median neurotomy can effectively alleviate spasticity of wrist and finger. The whole good nursing and rehabilitation guidance is the effective guarantee for success of operation and functional recovery of affected limb.
