摘  要:目的探讨初诊2型糖尿病(T2DM)、糖调节受损(IGR)及正常糖耐量(NGT)人群OGTT条件下血浆葡萄糖依赖性促胰岛素释放多肽(GIP)及胰高血糖素(GC)水平变化。方法选取16例NGT志愿者,13例IGR及19例初诊T2DM患者,三组均进行口服葡萄糖耐量试验(OGTT),测定0、30、60、90及120 min血浆GIP及GC浓度。结果三组受试者空腹血浆GIP水平[分别为NGT组(20.56±11.49),IGR组(27.04±13.07),T2DM组(20.21±10.44)]无显著差异,其余各时点血浆GIP水平T2DM组[分别为(131.52±43.30)、(153.53±41.53)、(147.72±38.69)、(123.29±36.04)]均较IGR组[分别为(188.26±34.23)、(200.88±18.11)、(190.31±33.32)、(172.91±27.94)]减少,差异有显著性。T2DM组GIP水平在30、90及120 min较NGT组[分别为(184.32±39.65)、(176.20±27.81)、(162.81±30.11)]减少,差异有显著性。T2DM组各时间点GC水平[分别为(102.05±9.10)、(91.37±7.60)、(94.72±8.69)、(82.47±7.15)、(76.21±8.15)]显著高于另外两组[NGT组分别为(63.16±7.89)、(59.15±7.53)、(57.08±7.64)、(55.81±7.60)、(53.11±7.40);IGR组分别为(76.00±3.05)、(71.59±4.45)、(59.16±5.59)、(63.17±5.27)、(58.07±4.73)],差异有显著性。结论初诊T2DM患者糖负荷后血浆GIP水平较NGT及IGR组下降,空腹及糖负荷后胰高血糖素均较NGT及IGR组升高。[ Objective ] To explore changes of Glucose-dependent Insulinotropic Polypeptide (GIP) and Glucagon (GC) in persons with newly diagnosed type 2 diabetes mellitus (T2DM), impaired glucose regulation (IGR) and nor- mal glucose tolerance (NGT). [Methods] 19 persons with newly diagnosed type 2 diabetes mellitus were selected as T2DM group. 13 persons with impaired glucose regulation were selected as IGR group.16 persons with normal glucose tolerance were selected as NGT group. OGTF test were performed in all the 3 groups and the levels of GIP and GC were measured at 0, 30, 60, 90 and 120 rain. [Results] The fasting plasma GIP levels among the 3 groups [NGT (20.56+11.49), IGR (27.04±13.07), T2DM (20.21±10.44)] showed no statistical difference. The levels of GIP in T2DM group [(131.52±43.30), (153.53±41.53), (147.72±38.69), (123.29±36.04)] were significantly lower than the levels of IGR group [(188.26±34.23), (200.88±18.11), (190.31±33,32), (172.91±27.94)] at the other time points. The levels of GIP in T2DM group were significantly lower than the levels of NGT group [(184.32±39.65), (176.20±27.81), (162.81±30.11)] at 30, 90 and 120 rain. The levels of GC in T2DM group [(102.05-+9.10), (91.37±7.60), (94.72±8.69), (82.47±7.15), (76.21±8.15)] were significantly higher than the levels of IGR group [(76.00±3.05), (71.59±4.45),(59.16±5.59), (63.17±5.27), (58.07±4.73)] and NGT group [(63.16±7.89), (59.15±7.53), (57.08±7.64), (55.81±7.60), (53.11±7.40)] at each time point. [ Conclusions ] The plasma GIP levels of the patients with newly diagnosed T2DM were significantly lower than the levels of persons with IGR and NGT after glucose load, while the plasma GC levels of the patients with T2DM were significanntly higher than the levels of persons with IGR and NGT at any time points.
