摘  要:目的:探讨保留股骨颈型人工全髋关节置换术治疗老年股骨头坏死的近期疗效。方法:收集因各种原因导致股骨头坏死的且在我中心行保留股骨颈人工全髋关节置换术的119例老年患者(133髋),分析其临床、手术和预后资料,探讨手术安全性及近期治疗效果。结果:所有133全髋关节置换术均顺利完成,手术时间185~305(225)min,术中出血量30~500(120)ml,围手术期输血仅5例,术后3级疼痛者12例,严重感染1例,控制感染后再次人工髋关节置换成功。经过6~75(44)个月随访,119例(133髋)人工全髋关节置换术后髋关节功能良好。影像学检查显示人工髋关节位置良好,假体无松动和下沉。股骨残端骨质较术前无明显变化。结论:保留股骨颈型人工全髋关节置换治疗老年人股骨头坏死近期疗效好,是治疗该病安全、有效的手术方法之一。Objectives: To investigate short-term efficacy of retained femoral neck total hip arthroplasty for femoral head osteonecrosis in older patients. Methods: One hundred and nineteen patients (133 hips) were under- went retained femoral neck total hip arthroplasty in our institute. The clinical data, operation information and fol- low-up data were recorded and analyzed. Results: All the 133 surgeries were successfully performed. The mean operation time and bleeding in this group of patients were 225min and 120ml, respectively. Peri-operative transfu- sion occurred in only 5 patients. Grade 3 pain happened in 12 patients. One severe infection was observed, and re- peat surgery was successfully conducted after the control of infection. After 6-75 months duration of follow-up, all the 133 artificial hips remained good function. X-ray and computer tomography showed that the location of the arti- ficial hips did not remove, and the bone of femur stump showed no dilapidate. Conclusion: Favorable short-term efficacy of retained femoral neck total hip arthroplasty for femoral head osteonecrosis in older population. It is an ef- fective and safe modality for management femoral head osteonecrosis in older patients.
