高同型半胱氨酸血症与高血压患者左室舒张功能的相关性

摘　　要：目的探讨高同型半胱氨酸(Hcy)血症与原发性高血压患者左室舒张功能不全的相关性。方法用循环酶法测定高血压患者80例[非左室肥厚组(NLVH)40例,左室肥厚组(LVH)40例]和40例健康者血清Hcy水平,并用超声心动图测量左室心肌质量指数(LVMI)、二尖瓣舒张早期流速峰值(EPFV)、晚期流速峰值(APFV)及峰值比(E/A)。结果对照组、NLVH组和LVH组血清Hcy水平分别为(9.17±1.68)、(19.52±2.76)、(25.93±3.93)μmol/L,E/A比值分别为(1.20±0.40)、(0.82±0.34)、(0.66±0.30),每两组比较差异均有统计学意义(P<0.01或0.05)。LVMI与血清Hcy水平呈正相关(r=0.482,P<0.01),E/A比值则与血清Hcy水平呈负相关(r=-0.413,P<0.01)。结论高Hcy血症与高血压患者的左室肥厚及左室舒张功能密切相关。Objective To investigate the association between hyperhomocysteinemia and left ventricular diastolic function in patients with hypertension. Methods Serum homocysteine （Hcy） level in 80 essential hypertension patients including 40 non- left ventricular hypertrophy （NLVH） and 40 left ventricular hypertrophy （LVH） cases and 40 healthy controls was detected by enzymatic cycling assay. LVMI, EPFV, APFV and E/A ratio were examined by echocardiogram. Results Serum Hcy level was （9.17± 1.68） μmol/L in control group, （19.52± 2.76） μmol/L in NLVH group and （25.93±3.93） μmol/L in LVH group, and significant difference was present between three groups （P〈0.01）. There were a positive correlation between LVMI and Hcy Ievel （r=0.482, P〈0.01） and a negative correlation between E/A ratio and Hcy content （r=-0.413, P〈0.01）. Conclusion Hyperho- mocysteinemia is closely related to left ventricular hypertrophy and diastolic function in hypertension patients.
