小针刀神经根管直接松解术治疗腰椎间盘突出症随机双盲对照临床研究

摘　　要：目的在前期研究基础上,进一步研究小针刀神经根管直接松解术治疗腰椎间盘突出症临床疗效及安全性。方法将50例腰椎间盘突出症患者按随机双盲对照法分为治疗组及对照组各25例,两组均给予针灸、推拿、牵引及中药辨证施治内服治疗,治疗组加载小针刀神经根管直接松解术,对照组加载神经根管假松解术。以无效、疗效不佳而要求结束试验为观察终点,试验中进行症状体征积分及MPQ量表评定、不良事件调查并记录,并随访半年。结果治疗组有效率明显优于对照组(P<0.05),两组总疗程差异无统计学意义(P>0.05),治疗组小针刀操作中均发生下肢串麻感,治疗及随访中未见神经损伤发生。结论小针刀神经根管直接松解术治疗腰椎间盘突出症具有显效快、疗效好、操作简便、痛苦小、潜在风险低的优势,值得推广。Objective To research further in the effect and security of the direct neurolysis on radicular canal with acupotomy to treat the lumbar disc protrusion on the basis of the preliminary study. Methods 50 cases of the lumbar disc protrusion were randomly divided into two groups：the treatment group and the control group. On the basis of both groups treating with the acupuncture,massage,traction and the traditional Chinese drugs based on differentiation,the patients of the treatment group were treated with direct neurolysis on radicular canal with acupotomy and the control group with the pseudo acupotomy. The study was end when the patients felt no effect or aggravated. All patients was evaluated with the cumulative score of the symptoms and signs,MPQ and adverse event during the treatment and visiting period. Return visit was lasted half a year after treatment. Results The effect of treatment group was obviously superior to that of the control group（P 〈 0.05）. The period of treatment had no significant difference between the two groups （P 〉 0.05）. Meanwhile,all of the patients in the treatment group occurred the electric feeling witch following the never distribution during the acupotomy therapy,but the potential risk of nerve damages was not detected. Conclusion The direct neurolysis on radicular canal with acupotomy treating the lumbar disc protrusion had better and faster efficacy,more convenience,less pain and less potential risk. It was worth spreading.
