妊娠相关血浆蛋白-A、β-HCG在异位妊娠早期诊断中的应用价值

摘　　要：目的检测可疑异位妊娠患者血清中β-HCG及妊娠相关血浆蛋白-A亚基SPP-A含量的变化,探讨其在异位妊娠早期诊断中的应用价值。方法选择2011年3月～2012年9月就诊于医院门诊的200例可疑异位妊娠患者为研究对象。采集其肘静脉血4mL,用酶联免疫法检测SPP-A含量,用化学发光法检测β-HCG,根据检测结果建立受试者工作曲线。结果异位妊娠组患者血清SPP-A含量低于宫内妊娠组,孕周不同SPP-A含量不同,差异均有统计学意义(P<0.05)。SPP-A诊断异位妊娠的ROC曲线下面积为0.901。SPP-A为1.470μg/mL时,诊断异位妊娠的灵敏度76.39%,特异度为85.71%,Youden指数为最大值0.621。结论妊娠相关血浆蛋白A可作为异位妊娠早期诊断的检测指标。Objective To detect the changes of serum β-HcG and pregnancy associated plasma protein-A subunit SPP-A in patients with suspected ectopic pregnancy,and explore the application value in early diagnosis of ectopic pregnancy. Methods The objects were 200 out-patients clinic cases of suspected ectopic pregnancy patients treated at out-patient clinic of the hospital from 2011 March to 2012 September.The elbow vein blood 4 mL of each case was collected.SPP-A were detected by enzyme immunoassay and chemiluminescence analysis was used to detect β -HCG. And then receiver operating curves were established according to the detection results. Results The SPP-A content of ectopic pregnancy group was lower than that of intrauterine pregnancy group.The SPP-A content was different with different pregnant weeks,and there were significant differences in both of them（P 〈 0.05）.Area under ROC curve of SPP-A in the diagnosis of ectopic pregnancy was 0.901. When SPP-A was 1.470 μg/mL,the diagnosis sensitivity of EP was 76.39%,specificity was 85.71%,Youden index were 0.621 as the maximum. Conclusion Pregnancy associated plasma protein-A can be used in the early diagnosis of ectopic pregnancy.
