海南地区不同人群TTV感染的流行病学研究

摘　　要：[目的 ]　研究海南地区不同人群TTV感染情况。 [方法 ]　对 162例城市一般人群、15 3例农村少数民族、12 0例献血员、96例丙氨酸氨基转移酶异常者、68例血透析者、92例非甲～庚型肝炎病人、10 2例甲～庚型肝炎病人、181例静脉毒瘾者 ,采用巢式PCR法检测血清中TTVDNA ,并对两株TTV的部分基因序列进行了测定。 [结果 ]　海南地区以上 8种人群中的阳性率依次为 10 5 %、8 5 %、9 2 %、11 5 %、17 6%、40 2 %、2 6 5 %、3 8 7%,各类人员之间差异非常显著 (P <0 0 1) ;TTV阳性者甲～庚型肝炎病毒合并感染率平均达 5 1 5 2 %,以HBV、HCV为常见 ,在2 7例合并感染中 ,急、慢、重型肝炎分别为 3 3 3 %、2 5 6%、40 7%,2 6例单纯TTV感染者无明显的肝炎表现。两株TTVDNA序列与日本株相应位置核苷酸序列的同源性大于 98%。 [结论 ]　海南地区不同人群TTV感染均常见 ,非甲～庚型肝炎、静脉毒瘾者是高危人群。Objective] To study TT virus infection among various populations in HaiNan.[Methods] The blood samples were from 8 different individuls groups. TTV DNA in serum sampies was detected by nested-polymerase chain reaction, partial gene of two strains of TTV was cloned and sequenced, and their genetic variation was analyzed.[Result] TTV DNA was detected in the sera from 17 of 162(10.5%) general individuals, 13 of 153(8.5%) countryside minority individuals, 11 of 120 (9.2%) blood donors, 11 of 96 (11.5%) elevated alanine aminotransferase persons, 12 of 68 (17.6%) hemodialysis patients, 37 of 92 (40.2%) patients with non-A to G hepatitis, 27 of 102 (26.5%) patients with A to G hepatitis, 7 of 181 (38.7%) intravenous drug users. There wrer significant different in various population (P<0 01). The sequence Analysis show that there were above 98% nucleotide identity among AB017610 (from Japan), AB011490 (from Japan).[Conclusion] The results of the study suggest that TTV infection is common in various populations in HaiNan. The intravenous drug users and patients with non-A to G hepatitis are the population at high risk for TTV infection.
