摘  要:目的探讨腘窝囊肿合并囊内游离体的低场MRI表现特征。方法回顾性分析临床证实的17例腘窝囊肿合并游离体的0.35T MRI资料。结果 17例患者18膝可见腘窝囊肿伴游离体。囊肿沿腓肠肌内侧头和半膜肌肌腱方向15个,沿腘肌腱2个,沿股二头肌腱1个。单囊囊肿14个,囊肿呈分叶状或多分隔状4个。囊肿大小:2.3 cm×1.5 cm×0.7 cm^6.8 cm×5.7 cm×3.9 cm。13膝显示囊肿通过腓肠肌和半膜肌的内侧头肌腱之间的细颈连接到膝关节,以轴位T2WI显示最佳。伴发的游离体数目:2~23个,平均(8.0±2.7)个。游离体大小:0.3~2.1 cm,平均(0.9±0.23)cm。游离体表现为T1WI及T2WI低信号、T1WI高T2WI高信号、T1WI高T2WI低信号等形式。术前MRI对游离体数目诊断的准确率为95.5%。伴膝关节积液14膝,MRI亦很好地显示伴发的膝关节其他病变。结论低场MRI对腘窝囊肿合并囊内游离体的诊断明确,又可观察其他膝关节病变,值得推广应用。[ Objective ] To investigate the characteristics of the popliteal fossa cyst with intracystic loose bodies in low-field MRI. [Methods] A retrospective analysis 0.35 TMRI data of 17 cases of popliteal fossa cyst with intra- cystic loose bodies confirmed by clinical. [Results] 18 knees involved in 17 patients. Along the medial head of gas- trocnemius and semimembranosus tendon in 15 cysts, 2 along the popliteal tendon, 1 along the femoral head tendon. Single sac cyst in 14, cysts were lobulated or separated like in 4. The cyst size: 2.3 cm×1.5 cm×0.7 cm-6.8 cm×5.7 crux3.9 cm. 13 knees showing the cyst connected to the knee through the narrow neck between the gastrocnemius muscle and the semimembranosus tendon, axial T2WI shows the best. Associated with the number of loose bodies: 2 to 23, average (8±2.1). Loose body size: 0.3 cm to 2.7 cm, average (0.9±0.23) cm. The signals of loose bodies in MRI: low signal in T1WI and T2WI, high signal in T1WI and in T2WI, high signal in T1WI and low signal in T2WI, and other signal forms. The diagnostic accuracy rate was 95.5% for preoperative MRI on the numbers of the loose bodies. With knee joint effusion in 14 knees, MRI also display associated with other lesions of the knee. [ Conclu- sion] It is clear for low-field MRI in the diagnosis of popliteal fossa cyst with intracystic loose bodies, with which other knee lesions can be observed, so it is worthy of popularization and application.
