男性2型糖尿病患者腰身指数与胰岛功能的关系

摘　　要：目的研究男性2型糖尿病(T2DM)患者腰身指数(WHtR)与胰岛功能的相关性。方法男性T2DM患者152例,以WHtR 0.5为切点分为非肥胖组和肥胖组,比较两组间的胰岛素抵抗指数(HOMA-IR)、胰岛素分泌指数(HOMA-IS)、胰岛素曲线下面积(AUCINS)、血糖曲线下面积(AUCBG)、血压、血脂等的异同。结果肥胖组较非肥胖组甘油三酯(TG)、VLDL-C、空腹胰岛素(FINS)、ln(AUCINS)、ln(HOMA-IR)和ln(HOMA-IS)升高,空腹血糖(FPG)和HbA1c降低,差异有显著性(P<0.05);Pearson相关分析显示WHtR与舒张压、TG、VLDL-C、FINS、ln(HOMA-IR)、ln(HOMA-IS)、Ln(AUCINS)正相关,P<0.01;与病程、HbA1c、FPG、AUCBG负相关,P<0.05;多元逐步回归分析显示,Ln(AUCINS)、TG、VLDL-C和FINS是WHtR的独立影响因素(P<0.01)。结论 WHtR有助于初步判断男性T2DM患者的胰岛功能和血脂状态。[Objective ] To investigate the relationship between waist height ratio （WHtR） and islet function in male type 2 diabetes mellitus （T2DM） patients. [Methods] With the cutpoint of WHtR at 0.5, 152 male T2DM pa- tients were divided into non-obesity group and obesity group. The differences in insulin resistance index （HOMA- IR）, insulin secretion index （HOMA-IS）, area under curve of insulin （AUC_INS）, area under curve of blood glucose （AUC_BG）, blood pressure, blood lipid and so on were compared. [ Results ] The obesity group have higher triglyceride （TG）, VLDL-C, fasting insulin （FINS）, In （AUC_INS）, In （HOMA-IR）, In （HOMA-IS）, but lower fasting plasma glucose （FPG） and HbAlc than non-obesity group. The differences have statistical significance （P 〈0.05）. Pearson correlation analysis indicate that WHtR is positively correlated with diastolic pressure, TG, VLDL-C, FINS, In （HOMA-IR）, in （HOMA-IS）, Ln（AUC_INS） （P 〈0.01）, but negatively correlated with course, HbAlc, FPG and AUCBG （P 〈0.05）. Multi- variant stepwise regression analysis revealed that in （AUC_INS）, TG, VLDL-C and FINS are main influent factors on WHtR （P 〈0.01）. [ Conclusion] WHtR may help initially judge islet function and blood lipid status in male T2DM patients.
