社区护理干预对女性神经源性膀胱患者生活质量的影响

摘　　要：目的探讨以自我间歇导尿和饮水控制为核心内容的社区护理干预对女性神经源性膀胱患者健康相关生活质量及并发症的影响。方法 2012年3～7月,采取整群抽样的方法收集吉首市5个社区53例女性神经源性膀胱患者;采用高校护理系、社区卫生服务中心、家庭联合,实施以定时自我间歇导尿和饮水计划为核心内容的社区护理干预模式,以WHO生存质量测定量表简表(WHOQOL-BREF)健康相关的生活质量评分为基础,增补神经源性膀胱常见并发症自制调查问卷进行调查,比较干预前后患者健康相关生活质量和导尿相关并发症发生状况。结果干预后本组神经源性膀胱患者总生活质量评分明显高于干预前,差异有统计学意义(P<0.01);干预后神经源性膀胱的典型并发症,如膀胱过度膨胀、尿失禁、尿潴留发生率显著低于干预前,差异有统计学意义(P<0.01);干预前后尿路感染发生率的差异无统计学意义(P>0.05)。结论高校护理系、社区卫生服务中心、家庭联合实施的以定时自我间歇导尿和饮水计划为核心的社区护理干预模式,有助于提高社区女性神经源性膀胱患者的生活质量,并减少常见并发症的发生率。Objective To evaluate the effect of community nursing intervention on the quality of life and complications in female patients with neurogcnic bladder. Methods Totally 53 female patients with neurogenic bladder were recruited in this study from 5 communities in Jishou City. Cooperated by nursing school, community health center and family members, a community nursing intervention program with a focus on the intermittent self-catheterization and drinking plan was implemented. The patients were investigated with the WHO Quality of Life Scale（WHOQOL-BREF） before and after the inter- vention. Results After 5-months intervention, the score of quality of life in patients with neurogenic bladder was significantly higher than before （P〈0.01）; the incidences of typical complications of neurogenic bladder including excessive expansion of the bladder, urinary incontinence, urinary retention were significantly reduced（P〈O.01）, while no significant difference was found bet- ween the incidences of urinary tract infections before and after intervention（P〉O.05）. Conclusion Community nursing intervention with a focus on the intermittent self-catheterization and drinking plan can improve the quality of life and reduce the incidence of complications in patients with neurogenic bladder.
