Treatment of Acromioclavicular Dislocation and Distal Clavicular Fracture with TiNi Shape Memory Alloy Clavicular Hook

摘　　要：目的:探讨镍钛合金记忆锁骨钩接骨板内固定治疗Posr3-6型急性重度肩锁关节脱位和(或)Neer2型锁骨远端骨折的临床疗效。方法:24例急诊重度肩锁关节脱位伴18例锁骨远端骨折患者,经采用镍钛合金记忆锁骨钩接骨板内固定及韧带修复治疗。结果:切口均一期愈合,无医源性损伤,24例全部随访,随访时间2年～3年半。按中国人民解放军临床疾病诊断依据及治愈好转标准和Chen wj疗效评定标准,优18例,占75%,良5例占20.83%,可1例占4.17%。总优良率95.83%。结论:镍钛合金记忆锁骨钩接骨板治疗肩锁关节脱位伴锁骨远端骨折,不需钢丝螺钉、克氏针等辅助材料,不需打洞钻孔,固定牢固可早期功能活动,防止肩关节僵硬,是治疗肩锁关节脱位伴锁骨远端骨折的有效新技术,新方法之一。Objective ：To discuss clinical efficacy of internal fixation treatment of Posr3 - 6 type acute and severe acromi- oclavicular dislocation and （or） Neer2 type distal clavicular fracture with TiNi shape memory alloy clavicular hook bone plate. Methods：Adopt TiNi shape memory alloy bone plate internal fixation and ligament repair for 24 acute and severe acromioclavicular dislocation cases accompanied with 18 distal clavicular fracture patients. Results： The incision of this group was healed completely and there is no iatrogenic injury. All 24 patients were provided with follow - up care for 2 years to 3 years and a half. Base on clinical disease diagnosis basis and cure ＆ improvement standard of Chinese People＇ s Liberation Army as well as Chen wj efficacy evaluation criteria, 18 cases have excellent efficacy, accounting for 75%, 5 cases have sound efficacy, accounting for 20. 83 % and only 1 case is passable, accounting for 4. 17%. The overall acceptance rate is 95.83%. Conclusions：Treatment of acromioclavicular dislocation and distal clavicular fracture with TiNi shape memory alloy clavicular hook book plate requires neither auxiliary material like steel wire, screw and Kirschner wire nor drilling, and the fixation can enable early functional activities and prevent shoulder joint stiffness, so this method is an effective new technique for treatment of acromioclavicula＇r dislocation and clavicular fracture.
