血胃泌素和胃酸变化在新生儿窒息后胃肠功能障碍中的意义

摘  要:目的探讨血中胃泌素(GAS)、胃酸分泌变化在新生儿窒息后发生胃肠功能障碍中的意义。方法对117例新生儿窒息引起胃肠功能障碍的59例患儿分别采用动态监测法测定分析其出生后1,3,7 d空腹静脉血GAS,胃液PH,并取同期本院收治的非窒息新生儿60例作为对照。结果①窒息后发生胃肠功能障碍患儿第1,3d血GAS显著低于无障碍组和对照组,其中尤以第1d为显著,有统计学意义(P<0.05);胃液pH值则第1d显著低于无障碍组和对照组,差异有统计学意义(P<0.05)。②重度窒息组第1、3 d血GAS明显高于轻度窒息组,胃液PH值低于轻度窒息组。结论窒息新生儿GAS和胃酸分泌异常可能与胃肠道功能紊乱有关,且窒息程度越严重,胃肠功能障碍发生率越高。Objective To explore secrete changes of the gastrin and gastric acid in gastrointestinal dysfunction after neo- natal asphyxia. Methods Retrospectively study and analyze 59 cases of 117 cases who were caused in a stifling, using a dynamic monitoring measurements and analysis of birth of 1,3,7 d, which their stomach empty stomach is a lotion fill up with gas,the blood pH, as a contrast to the 60 cases. Results After the 3d to fill up with gas in the blood and gastrointesti- nal dysfunction group and the less, especially for the significant and ld is especially statistical significance (P 〈 0. 05 ) ; The pH value was significantly lower than ld gastrointestinal dysfunction group and the difference has statistics meaning (P 〈 0. 05 ). Severe gastrointestinal dysfunction group 1,3 d blood is clearly higher than a mild fill up with gas stifled groups, the pH value is less than a mild stomach stifling. Conclusions Neonatal asphyxia who are filled up with gastrin and stom- ach acid is likely to discharge the functions of gastrointestinal confused, and the more serious the smothered degree, the more higher functional impairment rate.
