微创胆囊切除术与普通开腹胆囊切除术的对比研究

摘　　要：目的探讨腹腔镜、小切口等微创胆囊切除术与普通开腹胆囊切除术的优缺点。方法选取中国人民解放军第82医院2005年1月至2011年12月行胆囊切除的患者1 235例,其中524例患者采用腹腔镜胆囊切除术的方式,212例患者采用小切口胆囊切除术,499例患者用普通开腹胆囊切除术的手术方式。比较这3组患者的手术时间、术后疼痛使用镇痛剂情况、术后体温变化情况、胃肠功能恢复情况、术后并发症以及住院天数等。结果腹腔镜胆囊切除术比小切口胆囊切除术、普通开腹胆囊切除术能更好的恢复患者个方面的功能,差异显著(P<0.01)。小切口胆囊切除术比普通开腹胆囊切除术更好的恢复患者的功能,差异明显(P<0.01)。结论腹腔镜胆囊切除术比小切口胆囊切除术、普通开腹胆囊切除术对患者的创伤性小、患者恢复情况最好,小切口胆囊切除术比普通开腹胆囊切除术对患者的创伤性较小,患者恢复也较快。Objective To investigate the the advantages and disadvantages of laparoscopy, a small incision minimally in- vasive cholecystectomy and the common open cholecystectomy. Methods January 2005 to December 2011, 1235 cases of cholecystectomy patients in our hospital, of which 524 cases treated with laparoscopic cholecystectomy, 212 patients with small -incision cholecystectomy, 499 patients with ordinary open cholecystectomy surgery. These three groups of patients, operative time, postoperative pain, use of analgesics, postoperative body temperature changes, the recovery of gastrointes- tinal function, postoperative complications and hospital days. Results The laparoscopic cholecystectomy than small incision cholecystectomy ordinary cholecystectomy better able to restore the function of patients with a significant difference （P 〈 0. 01 ）. The function of patients of small - incision cholecystectomy better than the ordinary cholecystectomy restore,there were significant differences （P 〈 0. 01 ）. Conclusions Laparoscopic resection compared small -incision cholecystectomy, common open cholecystectomy patients with traumatic small, the patients recovered the best, small -incision cholecystectomy than the ordinary open gallbladder resection in patients with traumatic small, patients recover fairly quickly.
