摘 要:目的 研究多孔羟基磷灰石 (PorousHydroxylapatitePHA)生物陶瓷作为植骨替代材料在颈椎椎体次全切除术中的可行性。方法 将生物陶瓷块植入 8只犬的颈椎椎体之间 ,行形态学观察和生物力学测定。并在 12例颈椎椎体次全切除术中用PHA生物陶瓷块行植骨融合。结果 术后 6个月生物陶瓷块已与犬颈椎体紧密相嵌 ,陶瓷内部的大部分孔隙中均有新生骨组织生长 ,其生物力学性能明显改善 ,在临床研究中 ,12例患者手术后 2 4~ 38个月影像学检查显示PHA生物陶瓷块与颈椎体融合良好。结论 PHA生物陶瓷植入后能与周围的骨组织形成良好的骨性融合 ,其生物力学性能基本满足颈椎的力学要求 。Objective To research the feasibility of porous hydroxylaptite bioceramic (PHA) as a kind of bone grafting substitute in the cervical subtotal vertebrectomy. Methods The PHA blocks as bone grafting were implanted into the defect of vertebrae in 8 canines. The morphological observation and biomechanical test of the implanted PHA blocks were carried out. The bone grafting fusion was made using PHA bioceramic blocks in 12 patieats for the cervical subtotal vertebrectomy. Results The bioceramic tightly mounted with the cervical vertebral body of canines six months after operation. The new bone tissues grew in most pores of the PHA blocks. The biomechanical property of the implanted PHA blocks increased significantly. Patientsimage examinations of 24 38 months after operation showed that PHA bioceramic blocks mounted well with the cervical vertebral body.Conclusions PHA bioceramic blocks can tightly incorporate with the cervical vertebral body after implantation. Its biomechanical property can satisfy the dynamic demand of the cervical vertebrae. And it is a kind of promising substitute as the bone grafting in the cervical subtotal vertebrectomy.
