摘  要:目的探讨正常非妊娠育龄女性、正常晚期妊娠及早发型子痫前期女性体内血浆蛋白(PZ)、超敏C-反应蛋白(hs-CRP)及D-二聚体(D-D)的含量变化及其与早发型子痫前期的相关性及临床意义。方法收集早发型子痫前期患者(孕周28~34周)80例(早发子痫前期组)、孕周相同的正常晚期妊娠女性80例(正常孕晚期组)及正常的非孕期女性40例(非孕组)。测定PZ、hs-CRP、D-D水平及PT及APTT时间,比较各项指标水平的差异及其相关性。结果 (1)正常孕晚期组PZ、D-D、hs-CRP含量高于非孕组,而PT、APTT与后者相比缩短(P<0.05);(2)早发子痫前期组患者PZ含量低于正常孕晚期组而血清D-D、hs-CRP含量与后者相比升高(P<0.05);2组PT、APTT比较差异无统计学意义(P>0.05);(3)重度早发型子痫前期患者与轻度早发型子痫前期患者相比血浆中PZ降低、D-D及hs-CRP含量升高(P<0.05);(4)PZ与D-D及hs-CRP均呈负相关(P<0.05),D-D的变化与hs-CRP呈正相关(P<0.05)。结论 (1)正常晚期妊娠女性血浆PZ、D-D及hs-CRP含量升高,PT、APTT时间缩短,血液呈高凝状态,是机体预防产后出血的应激保护性反应。(2)患者血浆PZ降低和hs-CRP、D-D升高可能导致早发型子痫前期疾病的发生发展。Objective Through the study on the chonges of plasma protein Z(PZ),hs-CRP,D-dipolymer (D-D) contents in non-pregnant women in reproductive age, normal late pre^mt women and the women with early-onset preeclampsia in order to explore the clinical significance of these parameters and the correlations between these indexes and early-onset preeclampsia. Methods 80 patients with early-onset preeclampsia ( gestational weeks 28 - 34, preeclampsia group) ,80 normal late-pregnancy women ( gestational weeks 28 ~ 34 ,late pregnancy group) and 40 normal non-pregnant women ( non-pregnancy group) were enrolled in the study. The plasma levels of PZ, hs-CRP and D-D and the length of PT, APTT was deteeted. These parameters wele compared and statistically analyzed. Results The plasma levels of PZ, D-D, hs-CRP in late pregnancy group were significantly higher than those in non-pregnancy group,however, the length of PT, APTT was obviously shortened ( P 〈 0.05 ) ; the plasma levels of PZ in preeclampsia group were significantly lower than those in late pregnancy group ( P 〈 0. 05 ), while the plasma levels of D-D and hs-CRP were significantly higher than those in late pregnancy group ( P 〈0.05) ,and there was no significant difference in the length of Pl" and APTT between the two groups ( P 〉0.05 );the plasma levels of PZ in severe preeclampsia group were significantly lower than in mild preeclampsia group ( P 〈 0. 05 ), however, the plasma levels of D-D and hs-CRP in severe preeclampsia group were significantly increased,as compared with those in mild preeclampsia group ( P 〈0.05). The PZ,D-D and hs-CRP were negatively correlated each, however, D-D was positively correlated with hs-CRPo Conclusion The plasma levels of PZ, D-D, and hs-CRP in normal late-pregnancy women are obviously increased, while the length of PT and AFTT is shortened,with patien' s blood being hypereoagulable states, which may be the stress protective reaction for the prevention of postpartum hemorrhage.
