不同镇静策略对机械通气患者呼吸循环功能的影响

摘　　要：目的探讨不同镇静策略对机械通气患者呼吸循环功能的影响。方法将69例机械通气患者随机分为对照组(咪达唑仑组)和观察组(舒芬太尼联合咪达唑仑组)并进行每日唤醒,观察比较治疗72 h前后呼吸循环情况。结果治疗后两组患者呼吸频率、浅快呼吸指数下降、氧合指数升高,观察组改善更明显(P<0.05);两组心率、中心静脉压降低,平均动脉压升高,观察组心率降低程度更明显(P<0.05)。结论与单用咪达唑仑镇静相比,舒芬太尼联合咪达唑仑可更有效地改善机械通气患者呼吸循环功能,且联合用药对呼吸循环功能无明显抑制作用。Objective To investigate the effect of different sedation protocols on patients,respiratory and circulatory during mechanical ventilation.Methods 69 patients were divided into Midazolam group（control group）and Sufentanil associated with Midazolam group（observation group）randomly.Boh group were carried out daily wake-up.Observed two groups of patients with respiratory and circulatory before and after 72 h.Results After sedation,the respiratory rate and rapid-shallow-breathing index in two groups were decreased,oxygenation index were improved,the observation group improved more obviously（P〈0.05）.Heart rate,central venous pressure were decreased and mean arterial pressure increased after therapy in both groups,heart rate in observation group decreased more obviously（P〈0.05）.Conclusion Compared with single Midazolam,addition of Sufentanil to Midazolam showed satisfactory sedative efficacy and good safety in mechanically ventilated patient.
