摘 要:目的研究百草枯(PQ)致兔急性肺损伤(ALI)早期的病理生理改变。方法20只新西兰白兔随机分为对照组和百草枯组。百草枯组以35mg/kg剂量一次性腹腔注射百草枯,建立Au模型,对照组以等体积生理盐水注射。实验各组分别于建模2、4、6h时间点行320排CT扫描胸腔,得到灌注图像的CTP(CTPerfusion)参数[包括局部血流量(regionalbloodflower,rBF)、局部血容量(regionalbloodvolume,rBV)、毛细血管通透性(permeability surface,rPS)],测定血清血管内皮生长因子(VEGF)质量浓度。6h后用空气栓塞法处死动物,取肺组织行病理观察。结果分析百草枯组CTP参数和血清VEGF质量浓度,rBF和rBV随实验延长逐渐降低,rPS和血清VEGF质量浓度随时间延长逐渐升高,各个时间点与对照组比较差异均有统计学意义(P〈0.05)。病理观察显示,光镜下百草枯组炎细胞浸润明显,肺泡上皮细胞增生,弥漫性肺泡间隔增宽,可见灶性出血。结论PQ致兔Au早期时肺灌注不良,血清VEGF质量浓度升高,结合病理结果提示,Au早期时肺血管通透性增加。Objective To study the pathophysiotogical changes at the early stage of acute lung injury (ALI) induced by paraquat (PQ) in rabbits. Methods 20 New Zealand white rabbits were randomly divided into control group and PQ group. The rabbits in PQ group were treated with PQ at 35 mg/kg through intraperitoneal injection,then would be made the model of ALl . The rats in control group were treated with the same volume of saline. In experimental groups, chest was scanned by 320 - slice CT for CT perfusion (CTP) parameters including the regional blood flower ( rBF), the regional blood volume(rBV) and the permeability surface(rPS) , and the vascular endothelial growth factor (VEGF) mass concentration was measured in serum on 2, 4, 6 h respectively after building models. Animals were sacrificed by air embolism and the lung tissues were obtained for pathological examination after 6 h. Results The CTP parameters and the VEGF mass concentration were analyzed in PQ group, the rBF and the rBV were gradually reduced with time, the rPS and the VEGF mass concentration were gradually increased with time. All the data were significantly higher than in the corresponding control group ( P 〈 0.05 ). The main pathological changes of PQ group by microscopic examination were inflammatory granulocyte iafihration, alveolar epithelial cell proliferation, disseminated thickening of alveolar septa and local hemorrhages. Conclusion The perfusion of lung is harmful and the VEGF mass concentration in serum is increased, which may reveal the increase in pulmonary vascular permeability with the pathological results at the early stage of ALI induced by PQ.
