心力衰竭病人的左心室功能与心律失常的关系

摘　　要：目的 :研究充血性心力衰竭病人左心室射血分数与心律失常的关系。方法 :用超声心动图及 2 4h心电监护研究了 53例由缺血性心脏病 ( 33例 )或扩张性心肌病 ( 2 0例 )引起的充血性心力衰竭病人。并根据其心功能损害的程度分为两组 ,对比两组病人心律失常发生的类型和频率及抗心衰治疗后心律失常的变化。结果 :严重心律失常的发生与左心室的射血分数 (LVEF)相关 ,当LVEF <30 % ,严重心律失常的发生率明显高于LVEF >30 %的病人 ,左心室射血分数低的病人其左心室内径、左心室收缩及舒张末容积均明显高于LVEF >30 %的病人。结论 :心力衰竭病人的左心室收缩功能受损害的程度直接影响着心律失常的类型及发生频率 ,左心功能受损严重者 ,其严重心律失常的发生率高 ,是引起心衰病人猝死的原因之一。Aim: To study the relationship between left ventricular function and arrhythmia in congestive heart failure. Methods: Fifty-three patients with congestive heart failure caused by coronary heart disease and dilated myocardiopathy were studied by using echocardiography and twenty-four hours ECG monitor according to the damaged degree of heart function. The patients were classified into two groups, studying their pattern, and frequency of arrhythmia ,obseive the change after treatment of the congestive heart failure. Results: The fraction of left ventricular ejection was related to the occurance of obstinate arrhythymia, when LEVF was less than 30%, the occurance of malignant arrhythymia was higher than that of LEVF >30%, The curative effect of treatment against arrythmia was less in patients with lower LEVF than patients with higer LEVF. The left ventricular internal diameter, end-systolic and end-diastolic volume in patients with lower LEVF was larger than that in the patients with higher LEVF. Conclusion: Left ventricular systolic function was directly related to the pattern and frequency of arrythmia, when the left ventricular function was damaged severely, the frequency of malignant arrhythmia was higher. It was also one of the causes of sudden death in patient of the heart failure. [
