甲状腺功能亢进症及甲亢性心脏病患者血浆心钠素变化的临床观察

摘　　要：目的　观察甲状腺功能亢进症 (甲亢 )及甲亢性心脏病 (甲亢心 )患者治疗前后血浆心钠素 (ANH)的变化及其临床意义。方法　 2 6例甲亢及 18例甲亢心患者于治疗前及治疗 4周后分别抽血测血浆ANH及甲状腺功能相关指标 ,并与正常对照组比较。结果　①甲亢和甲亢心患者的血浆ANH治疗前、后与正常对照组比较 ,差异均有高度显著性(P <0 0 1) ;治疗前与治疗后ANH比较 ,差异也有高度显著性 (P <0 0 1)。②单纯甲亢与甲亢心两组患者治疗前及治疗后ANH比较 ,差异均有高度显著性 (P <0 0 1)。结论　甲亢及甲亢心患者血浆ANH浓度升高 ,且随病情好转而下降 ,ANH明显升高的甲亢患者要警惕甲亢心的可能。Objectives To study the change of Plasma atrial natriuretic hormone (ANH) in hyperthyroidism and hyperthyroid heart disease. Methods Fasting plasma concentrations of ANH and thyroid function tests were examined by RIA in 26 patients with hyperthyroidism and 18 with hyperthyroid heart disease and 40 healthy controls, all the hyperthyroid patients were investigated before and after 4 weeks treatment with antithyroid drugs. Results (1)Compared with healthy controls, all the hyperthyroid patients had significantly increased ANH ( P <0 01) before and after treatment, there were markedly decreased ANH concentrations ( P <0 01) after treatment; (2)Compared with hyperthyroid patients, the patients with hyperthyroid heart disease had markedly increased ANH (P<0 01) before and after treatment. Conclusion Patients with hyperthyroidism and hyperthyroid heart disease have increased plasma ANH and it could been reduced after treatment with antithyroid drugs, the higher ANH in hyperthyroid patient indicate the patient may combine hyperthyroid heart disease.
