摘 要:目的:探讨分析经颅多普勒超声(TCD)在急性期脑梗死诊断中的应用。方法:回顾性分析2009年8月-2011年5月我院收治的86例经头颅CT或MRI确诊的急性脑梗死患者,应用TCD进行脑血管血流动力学监测。并与正常对照组(A组)45例进行比较。结果:86例急性脑梗死患者(B组)的TCD检查结果发现病灶区血流主要表现为减慢、加快或消失,其中82例脑血流异常,异常率95.35%,血流速度减慢39例,占44.35%;血流速度加快36例,占41.86%,36例血流速度加快有29例为脑血管狭窄,7例为血管痉挛;血流消失7例,占8.13%。A组血流异常率为33.4%,B组血流异常率明显高于A组,差异有统计学意义(P<0.05)。结论:TCD具有无创、实用、经济、简便等优点,可用于急性期脑梗死患者了解颅内血管血流动力学及血管阻力,间接了解颅内压,为治疗及预后提供参考。Objective: To investigate the application of transcranial Doppler (TCD) in the diagnosis of acute cerebral stroke. Methods: We retrospectively analyzed the profiles of 86 patients( Group B)with transcranial CT- or MRI-diagnosed acute cerebral stroke who were admitted to the department of emergency, The Second Affiliated Hospital of Guangzhou Medical College, between August 2009 and May 2011. TCD was employed to determine the cerebral vascular hemodynamics for comparison with that of 45 normal controls ( Group A ). Results: The major presentations of acute cerebral stroke comprised of slowed, accelerated and stagnated blood flow at the affected side. 82 cases (95.35%) were featured by aberrant cerebral blood flow, 36 (44.35%) showed slowed blood flow and 36 (41.86%) presented accelerated blood flow. The diagnosis included cerebral vascular stenosis ( n = 29) , vasospasm ( n = 7 ) and absence of blood flow ( n = 7, 8.13% ). Group A yielded a remarkably lower rate ( 33.4% ) of aberrant blood flow ( P 〈 0.05 ). Conclusion: Featured by non-invasiveness, practicability, low cost and convenience, TCD may be applied to determine the intraeranial vascular hemodynamics and resistance as well the intraeranial pressure, thus offering rationale for the treatment and prevention of acute cerebral stroke.
